ارتباط عوامل ساختاری و بینابینی تعیین کننده های اجتماعی سلامت با وزن کم هنگام تولد در ایران: مرور سیستماتیک و متاآنالیز

زمینه و هدف: تجارب اولیه دوره داخل رحمی برای جنین، پایه ی حیاتی تمام دوران زندگی بوده؛ بنابراین توجه به آن می تواند منجر به کاهش بی عدالتی سلامت در درون یک نسل را فراهم نماید. هدف از پژوهش حاضر، تعیین ارتباط بین عوامل ساختاری و بینابینی تعیین کننده های اجتماعی سلامت با وزن کم هنگام تولد در ایران بود.
روش بررسی: این مطالعه مرور سیستماتیک و فراتحلیل بر اساس مطالعات انجام شده در ایران با جستجوی پیشرفته در پایگاه های اطلاعاتیIranmedex, Magiran, SID, Irandoc PubMed,  ،Google Scholar و Scopus و با استفاده از کلید واژه های تعیین کننده های اجتماعی سلامت، وزن کم هنگام تولد و معادل انگلیسی آنها از بین کلیه مقالات سالهای2000- 2016 تدوین گردید، نتایج مطالعات مرتبط توسط دو نفر به طور مجزا استخراج و با استفاده از چک لیست استروپ مورد ارزیابی قرار گرفتند. سپس داده ها با استفاده از مدل تصادفی با هم ترکیب شده و ناهمگنی مطالعات با استفاده از شاخص I2 بررسی و آنالیز داده ها نیز با استفاده از نرم افزار STATA نسخه 11 انجام شد.
یافته ها: در این مرور سیستماتیک و فراتحلیل 30 مقاله وارد آنالیز شدند. تعداد کل نمونه در مطالعه حاضر42142 نفر بودند. نسبت شانس کلی برآورد شده برای ارتباط عوامل ساختاری و بینابینی تعیین کننده های اجتماعی سلامت با وزن کم هنگام تولد52/1 (85/1-24/1 درصد: فاصله اطمینان 95درصد) و24/2 (71/2-85/1 درصد: فاصله اطمینان 95درصد) بود.
نتیجه گیری: با توجه به اهمیت وزن هنگام تولد در سلامت نوزادان باید اقدامات اساسی در جهت پیشگیری از وقوع این پیامد مهم از طریق شناسایی افراد در معرض خطر و عوامل موثر در بروز آن، بهبود استاندارد های زندگی، افزایش مراقبت های منظم پره ناتال، مشاوره های قبل از بارداری و بهبود وضعیت تغذیه ای زنان باردار صورت گیرد.
