ASSESSMENT OF THALAMIC METABOLITE CHANGES IN PATIENTS WITH IDIOPATHIC GENERALIZED EPILEPSY USING PROTON MAGNETIC RESONANCE SPECTROSCOPIC IMAGING (H1-MRSI)

Background and Aim: Idiopathic Generalized Epilepsy (IGE) is a type of epilepsy characterized primarily by generalized seizures. Magnetic Resonance Spectroscopy (MRS) is a non-invasive imaging technique capable of detecting metabolic alterations in the brain. A hallmark of metabolic abnormalities in seizure disorders is a reduction in N-acetyl aspartate (NAA). This study aimed to investigate thalamic abnormalities in patients with IGE using MRS.
Materials and Methods: This study was performed on 10 patients diagnosed with IGE and 10 healthy subjects serving as the control group, using the MRS Chemical Shift Imaging (MRS-CSI) technique. Patients and controls were matched for age and gender. A multivoxel MRS protocol was applied to the region of interest. Data analysis was carried out using the independent samples t-test.
Results: The findings demonstrated that the NAA/Cr ratio in the thalamus of patients with IGE was abnormally lower than that of healthy controls. However, no significant differences were observed in the NAA/Cr ratio within the insular cortex or the corpus callosum. Furthermore, the t-test analysis revealed no statistically significant difference in the NAA/Cho ratio between the patient and control groups.
Conclusion: The present study suggests that the thalamus may play a role in the pathophysiology of IGE. The observed reduction in the NAA/Cr ratio may serve as a metabolic marker for identifying neuronal abnormalities associated with this disorder. These findings support the value of MRS as a useful tool in the clinical investigation of epilepsy.
