fa بررسی شیوع آلرژن های غذایی در بیماران مبتلا به درماتیت آتوپیک مراجعه کننده به کلینیک آسم و آلرژی در تهران در سال1393 Prevalence of food allergens in patients with atopic dermatitis referring to the asthma and allergy clinic in Tehran in 2014 عمومى General پژوهشي اصیل Original Research مقدمه: درماتیت آتوپیک یک بیماری شایع مزمن التهابی است که پوست را تحت تاثیر قرار می دهد و یکی از عوامل مهم در آغاز و افزایش شدت این بیماری، آلرژی غذایی است. شناخت الگوی حساسیت به انواع آلرژن های غذایی در جامعه، می تواند نقش مهمی در آگاهی قبلی و پیشگیری از عود و تشدید بیماری درماتیت آتوپیک داشته باشد. هدف این مطالعه بررسی میزان شیوع آلرژن های شایع غذایی در افراد مبتلا به درماتیت آتوپیک با استفاده از تست پوستی پریک در شهر تهران در سال 1393می باشد. روش بررسی: در یک مطالعه توصیفی در تهران، تمام افراد مراجعه کننده به کلینیک تخصصی آسم و آلرژی خورشید، طی سال 1393 معاینه شده و در صورت ابتلا به آلرژی انتخاب و وارد مطالعه شدند. پس از تشخیص موارد بالینی درماتیت آتوپیک در این افراد، تست پوستی پریک در مورد آلرژن های شایع غذایی انجام گرفت. <span lang="FA" style="font-family: "B Zar"; font-size: 12pt; mso-fareast-font-family: "Times New Roman"; mso-bidi-language: FA; mso-ascii-font-family: "Times New Roman"; mso-hansi-font-family: "Times New Roman";">در نهایت داده ها توسط نسخه 20 نرم افزار  SPSS تجزیه و تحلیل شدند. یافته ها: از بین 1012 فرد مبتلا به آلرژی، 282 نفر (حدود 28%) مبتلا به درماتیت آتوپیک تشخیص داده شدند. از این تعداد، 87 نفر (31 % افراد، 52 زن و 35 مرد) حداقل به یکی از آلرژن های غذایی حساس (تست پریک مثبت) بودند. در افراد مبتلا به درماتیت آتوپیک دارای آلرژی غذایی، شیوع آسم، رینیت آلرژیک و کهیر به ترتیب 8/6، 4/11 و 5/4 درصد بود. بین جنسیت و آلرژی غذایی در افراد مبتلا به درماتیت آتوپیک ارتباطی وجود نداشت. شایع ترین آلرژن های غذایی در بیماران به ترتیب زرده تخم مرغ (4/38 %)، سفیده تخم مرغ (5/36 %)، فندق (3/33 %) و بادام زمینی (7/28 %) بوده و کمترین شیوع در برنج (6/4 %)، جو(7/5 %) و گوشت گوسفند (9/6 %) وجود داشت. نتیجه گیری: شیوع درماتیت آتوپیک در بیماران آلرژی این مطالعه بیشتراز حد انتظار بود. حساسیت به تخم مرغ و انواع آجیل به عنوان شایع ترین آلرژن ها، به ویژه در مورد کودکان، باید مورد توجه متخصصین آلرژی و نیز خانواده ها قرار گیرد. کلمات کلیدی: درماتیت آتوپیک، آلرژی غذایی، تست پوستی پریک. <span dir="RTL" lang="FA" style="line-height: 115%; font-family: "B Zar"; font-size: 13pt; mso-fareast-font-family: Calibri; mso-bidi-language: FA; mso-ansi-language: EN-US; mso-fareast-language: EN-US; mso-ascii-font-family: Calibri; mso-hansi-font-family: Calibri;">وصول مقاله:25/6/94 اصلاحیه نهایی:30/8/94 پذیرش:19/10/94 Background and Aim: Atopic dermatitis is a common chronic inflammatory disease the skin. Food allergy is one of the main factors affecting the onset and severity of the disease. Recognition of the pattern of sensitivity to food allergens in the community can play an important role in the public awareness and prevention of recurrence of atopic dermatitis. The aim of this study was to recognize the prevalence of common food allergens in the patients with atopic dermatitis by means of skin prick test in Tehran during 2014. Material and Methods: In this cross sectional study, we examined all the patients for the signs and symptoms of allergies in the Asthma and Allergy Clinic during 2014. Skin prick test was performed for common food allergens in the patients with atopic dermatitis . SPSS version 20, was used for data analysis. Results: Out of 1012 patients with allergy, 282 cases (approximately 28%) were diagnosed as atopic dermatitis. 87(52 female and 35 male) patients with atopic dermatitis, were sensitive to at least one food allergen (positive prick test). In patients with atopic dermatitis the prevalence rates of asthma, allergic rhinitis and urticaria were 6.8, 11.4 and 4.5 %, respectively. There was no relationship between gender and food allergy in the patients with atopic dermatitis. The most prevalent food allergens were egg yolk (38.4%), egg white (36.5%), hazelnut (33.3%), and peanut (28.7%), and the least prevalent allergens were found in rice (4.6%), barley (5.7%), and meat (6.9%). Conclusion: The prevalence rate of atopic dermatitis in allergic patients in this study was more than expected. Sensitivity to egg and nuts as the most common allergens should be taken into consideration by families and allergy specialists.
