style="font-family: "B Zar" font-size: 12pt text-underline: none"> سردرد یکی از شایع ترین علل مراجعه بیماران به پزشکان می باشد و اغلب افراد جامعه حداقل یک بار حمله سردرد شدید را در طول عمر خود تجربه کرده اند. این مطالعه با هدف شناخت میزان شیوع</span><span dir="LTR" lang="FA" style="font-size: 12pt mso-bidi-font-family: "B Zar" text-underline: none"><font face="Calibri"> </font></span></font><span lang="FA" style="font-family: "B Zar" font-size: 12pt text-underline: none"><font color="#000000">سردرد های برق آسا و </font><span style="color: black">علل</span><font color="#000000"> آن در سنندج صورت گرفت. </font></span></p> <p></p> <p dir="RTL" style="margin: 0in 0in 0pt line-height: normal text-indent: 0in"><font color="#000000"><b><span lang="FA" style="font-family: "B Zar" font-size: 12pt text-decoration: none text-underline: none">روش بررسی:</span></b><span lang="FA" style="font-family: "B Zar" font-size: 12pt text-underline: none"> در این مطالعه توصیفی-تحلیلی، تمامی بیماران با شک به سردرد های حاد و ناگهانی در طی سالهای 1393-1392 که در اورژانس بیمارستان توحید سنندج بستری شده بودند، با روش سرشماری بررسی شدند. پس از جمع‌آوری داده ها از پرونده بیماران و نیز پرسشنامه با استفاده از آمارهای توصیفی، توصیف داده‌ها انجام شد. با استفاده از نرم افزار </span><span dir="LTR" style="font-size: 12pt mso-bidi-font-family: "B Zar" text-underline: none"><font face="Calibri">SPSS</font></span><span lang="FA" style="font-family: "B Zar" font-size: 12pt text-underline: none"> و تستهای آماری تی، کای دو، فیشر<span style="mso-spacerun: yes">  </span>و رگرسیون لجستیک تحلیل انجام گردید.</span></font></p> <p></p> <p dir="RTL" style="margin: 0in 0in 0pt line-height: normal text-indent: 0in"><b><span lang="FA" style="color: black font-family: "B Zar" font-size: 12pt text-decoration: none text-underline: none">یافته‌ها: </span></b><font color="#000000"><span lang="FA" style="font-family: "B Zar" font-size: 12pt text-underline: none">از نظر محل سردرد، تعداد 43 نفر (1/36%) سردرد در ناحیه فرونتال و 24 نفر (2/20%) سردرد در تمام سر داشتند. تعداد 58 نفر (7/48%) بطورکلی دارای ضایعه فضاگیر مغزی بودند. تعداد 47 (5/39%) نفر دارای سردرد رعدآسا بودند. در کل افراد دارای سردرد شدید، تعداد 30 نفر (2/25%) خونریزی زیر عنکبوتیه (</span><span dir="LTR" style="font-size: 12pt mso-bidi-font-family: "B Zar" text-underline: none"><font face="Calibri">SAH</font></span><span lang="FA" style="font-family: "B Zar" font-size: 12pt text-underline: none">) داشتند که شیوع آن در گروه دارای سردرد رعدآسا بالا بود (4/57%). نتیجه رگرسیون لجستیک نشان داد که شروع سردرد زیر یک هفته شانس داشتن ضایعه</span><span dir="LTR" lang="FA" style="font-size: 12pt mso-bidi-font-family: "B Zar" text-underline: none"><font face="Calibri"> </font></span><span lang="FA" style="font-family: "B Zar" font-size: 12pt text-underline: none">مغزی را 657/11 برابر (027/0=</span><span dir="LTR" style="font-size: 12pt mso-bidi-font-family: "B Zar" text-underline: none"><font face="Calibri">p</font></span><span lang="FA" style="font-family: "B Zar" font-size: 12pt text-underline: none">)، تشدید سردرد با سرفه یا عطسه حدود 42 برابر (006/0=</span><span dir="LTR" style="font-size: 12pt mso-bidi-font-family: "B Zar" text-underline: none"><font face="Calibri">p</font></span><span lang="FA" style="font-family: "B Zar" font-size: 12pt text-underline: none">) و عدم وجود سابقه قبلی سردرد نیز شانس داشتن ضایعه را حدود 6/5 برابر بیشتر خواهد کرد (052/0=</span><span dir="LTR" style="font-size: 12pt mso-bidi-font-family: "B Zar" text-underline: none"><font font-family: "B Zar" font-size: 12pt text-underline: none">طبق نتایج</span><font color="#000000"><span lang="FA" style="font-family: "B Zar" font-size: 12pt text-underline: none"> این مطالعه، شروع سردرد زیر یک هفته و تشدید سردرد با سرفه، عطسه و عدم وجود سابقه قبلی، با شانس بیشتر ضایعه فضاگیرمغزی</span><span dir="LTR" lang="FA" style="font-size: 12pt mso-bidi-font-family: "B Zar" text-underline: رعدآسا میدانند، در بخش اورژانس خونریزیهای اینتراکرانیال شامل: </span><span dir="LTR" style="font-size: 12pt mso-bidi-font-family: "B Zar" text-underline: none"><font Using</font><span style="color: black"> SPSS</span><font color="#000000"> version 18, data were analyzed by descriptive statistics</font><span style="color: black">, T-test, </span></span><em><span style="background: white font-size: 12pt text-underline: none"><font color="#000000" face="Calibri">Fisher&#39s exact test</font></span></em><span style="color: black font-family: "Times New Roman","serif" font-size: 12pt mso-fareast-font-family: Calibri text-underline: none">, Chi-square, and Logistic regression.</span></p> <p></p> <p style="margin: 0in 0in 0pt text-align: justify line-height: normal text-indent: 0in unicode-bidi: embed direction: ltr"><font color="#000000"><b><span style="font-family: "Times New Roman","serif" font-size: 12pt text-decoration: none mso-fareast-font-family: Calibri text-underline: none">Results:</span></b><span style="font-family: "Times New Roman","serif" font-size: 12pt text-underline: none"> </span><span style="font-family: "Times New Roman","serif" font-size: 12pt mso-fareast-font-family: Calibri text-underline: none">Based on the sites of the headache, 43 (36.1%) and 24 (20.2%), patients had frontal headache and generalized headache, respectively. 58 patients (48.7%) had a space-occupying lesion in the brain. 47 (39.5) patients had thunderclap headache. Among the patients with severe headache, 30 (25.2%) patients had subarachnoid hemorrhage, which was more common in the group with thunderclap headache. Results of logistic regression showed that risk of having brain injury in the patients suffering from <span style="mso-spacerun: yes"> </span>headache of less than one week duration increases up to 11.657 times (P=0.027).This risk increases in the patients with headache aggravated by coughing or sneezing and headache with no previous history to 42 times (P=0.006) and 5.6 times <span style="mso-spacerun: yes"> </span>respectively (P=0.052).</span></font></p> <p></p> <p style="margin: 0in 0in 0pt text-align: justify line-height: normal text-indent: 0in unicode-bidi: embed direction: ltr"><font color="#000000"><b><span style="font-family: "Times New Roman","serif" font-size: 12pt text-decoration: none mso-fareast-font-family: Calibri text-underline: none">Conclusion:</span></b><span style="font-family: "Times New Roman","serif" font-size: 12pt mso-fareast-font-family: Calibri text-underline: none"> Headaches aggravated by coughing, sneezing, and with no previous history were associated with higher incidence of space-occupying lesions (brain tumor, S.A.H,…). Also subarachnoid hemorrhage (SAH) and meningitis were two major causes of thunderclap headache. Other studies showed that migraine is the most common cause of thunderclap headache, but in emergency wards, intracranial hemorrhages including SAH & ICH were more common causes of this type of headaches.
