fa اختلال عملکرد اسفنکتر مقعدی با بی‌اختیاری مدفوع ناشی از اسکار حلقوی سوختگی اطراف مقعد، گزارش موردی عارضه غیرمعمول سوختگی پرینه عمومى General پژوهشي اصیل Original Research زمینه و هدف: سوختگی عمقی ناحیه پرینه به تنهایی نادر می‌باشد و در مجموعه سوختگی سطح وسیعی از بدن معمولاً دیده می‌شود و تمام علل سوختگی اعم از حرارتی، شیمیایی و الکتریکی دخالت دارند. درمان اغلب موارد محافظه کارانه بوده و دخالت جراحی به ندرت لازم می‌شود. اما سوختگی ایزوله و عمقی ناحیه پرینه در موارد خاصی از حالت وقوع مثل افتادن داخل ظرف حاوی مواد سوزاننده، تنور و نشستن روی اگزوز وسیله نقلیه گزارش شده اما اختلال عملکرد اسفنکتر مقعدی به دنبال اسکار حلقوی سوختگی اطراف مقعد گزارش نشده است. هدف ما توجه به موارد خاص وقوع سوختگی ایزوله پرینه و عوارض آن است. معرفی بیمار: یک پسر بچه 12 ساله افغانی که در سن 6 ماهگی دچار سوختگی ناحیه پرینه ناشی از افتادن داخل دیگ آب جوش شده که با درمان محافظه‌کارانه مراقبت شده و از زمانی که والدین او به یاد دارند بی‌اختیاری مدفوع داشته و در معاینه هم اسکار دورتادوری مقعد و عدم تونیسیته اسفنکتر وجود داشته که با درمان جراحی ریلیز اسکار و گرافت پوستی تون اسفنکتر تدریجاً برگشت نمود. نتیجه‌گیری: در وقوع موارد خاص سوختگی پرینه که آسیب عمقی و ایزوله دیده می‌شود. درمان کنزرواتیو و جوش خوردن خود به خودی با ایجاد اسکار احتمال اختلال در عملکرد اسفنکتر مقعدی و بی‌اختیاری مدفوع که از نظر جسمی، روانی و اجتماعی به ویژه در اطفال ناتوان‌کننده است وجود دارد، با درمان به موقع و مناسب می‌توان از این عوارض جلوگیری کرد. وصول مقاله: 18/5/85 اصلاح نهایی: 2/6/85 پذیرش مقاله: 4/6/85 Background and Aim: Isolated deep burn of perineal area is rare and usually occurs in cases with extensive burn. The etiology of this kind of burn may be of thermal, chemical or electrical origin. Treatment in most cases is conservative and surgical intervention rarely required. Isolated deep burn of perineal area has been reported in some special circumstances such as falling into furnace or pots containing hot fluids and sitting on exhaust pipe of vehicles but no dysfunction of anal sphincter after formation of perineal annular scar had been detected. Our objective is to focus on particular cases of perineal burn and its complications. Case Report: A 12 years old Afghan boy with history of previous burn of perineal area at the age of 6 months, due to falling into boiled water, referred to Tohid Hospital with chief complaint of fecal incontinence. At that time the patient had been managed conservatively for his burn. As far as the patient`s parents remember, he had been suffering from fecal incontinence since his childhood. In physical exam an annular scar around anus together with lack of sphincter tone were detected. By releasing the scar and use of skin graft, sphincter tone improved gradually. Conclusion: In special cases of isolated perineal deep burn, conservative management may lead to scarring and dysfunction of anal sphincter with fecal incontinence, imposing physical, social and psychological impacts specially on children.
