fa بررسی فراوانی واریکوسل در مردان نابارور مبتلا به آزواسپرمی غیر انسدادی مراجعه کننده به مرکز ناباروری سنندج در سال 1400-1399 Investigate frequency of varicocele in infertile men with non-obstructive azoospermia referred to Sanandaj Infertility Center in 2020-2022 پزشکی- اورولوژی Medicine - Urology كاربردي Applicable چکیده  زمینه و هدف:  واریکوسل به عنوان اتساع و پیچ خوردگی غیرطبیعی وریدهای شبکه پامپینیفرم در طناب اسپرماتیک تعریف می‌شود. این بیماری تقریباً در 15٪ از جمعیت عمومی مشاهده می‌شود. علیرغم شیوع نسبتاً بالای آن و ارتباط اثبات شده آن با اختلال در اسپرماتوژنز و استروئیدوژنز، تأثیر خاص آن بر عوامل شدید ناباروری مردان نیاز به بررسی بیشتر دارد. این مطالعه با هدف تعیین شیوع واریکوسل در مردان نابارور مبتلا به آزواسپرمی غیر انسدادی که بین سال‌های 1399-1400 به مرکز ناباروری سنندج ارجاع داده شده بودند، انجام شد. مواد و روش ها: این مطالعه مقطعی (توصیفی-تحلیلی) بر روی ۱۷۰ مرد نابارور مبتلا به آزواسپرمی غیرانسدادی در مرکز ناباروری سنندج انجام شد. داده‌های بیمارانی که پرونده پزشکی آنها موجود بود و تشخیص ناباروری آزواسپرمی در آنها تایید شده بود، مورد تجزیه و تحلیل قرار گرفت. تشخیص آزواسپرمی بر اساس شرح حال جامع بیمار، معاینه بالینی و آزمایش‌های آزمایشگاهی بود. سطح معنی‌داری برای این مطالعه ۵٪ در نظر گرفته شد. نتایج : نتایج نشان داد که در میان بیماران مبتلا به واریکوسل، 14 نفر (14.1%) واریکوسل دو طرفه، 14 نفر (14.1%) واریکوسل سمت راست و 59 نفر (67.8%) واریکوسل سمت چپ داشتند. در مورد درجه واریکوسل، 25 بیمار (28.7%) درجه I، 34 نفر (39.1%) درجه II و 28 نفر (32.2%) درجه III داشتند. در مجموع، 72 نفر (42.4%) سابقه سیگار کشیدن و 26 نفر (15.3%) سابقه مصرف الکل را گزارش کردند. بحث :  یافته‌های این مطالعه نشان می‌دهد که متغیرهای محیطی مانند سیگار کشیدن و مصرف الکل، همراه با سابقه خانوادگی واریکوسل و عوامل جمعیت‌شناختی مانند سن و شغل، ممکن است با شیوع واریکوسل مرتبط باشند. با این حال، اپیدمیولوژی واریکوسل هنوز به طور کامل شناخته نشده است. بنابراین، مطالعات در مقیاس بزرگ و به خوبی طراحی شده که شامل ارزیابی عوامل بالینی کلیدی باشند، برای روشن شدن کامل جنبه‌های اپیدمیولوژیک این بیماری ضروری است ABSTRACT Background and Aim: Varicocele is defined as the abnormal dilatation and tortuosity of the pampiniform plexus veins within the spermatic cord. It is observed in approximately 15% of the general population. Despite its relatively high prevalence and its established association with impaired spermatogenesis and steroidogenesis, its specific impact on severe male infertility factors requires further investigation. This study aimed to determine the prevalence of varicocele in infertile men diagnosed with non-obstructive azoospermia who were referred to the Sanandaj infertility center between 2020 and 2022. Materials and methods: This cross-sectional (descriptive-analytical) study was conducted on 170 infertile men diagnosed with non-obstructive azoospermia at the Sanandaj infertility center. Data from patients with available medical records and a confirmed diagnosis of azoospermic infertility were analyzed. The diagnosis of azoospermia was based on comprehensive patient history, clinical examination, and laboratory tests. A significance level of 5% was considered for this study. Results: The results indicated that among the patients with varicocele, 14 individuals (14.1%) presented with bilateral varicocele, 14 (14.1%) with right-sided varicocele, and 59 (67.8%) with left-sided varicocele. Regarding varicocele grade, 25 patients (28.7%) had grade I, 34 (39.1%) had grade II, and 28 (32.2%) had grade III. Overall, 72 participants (42.4%) reported a history of smoking, and 26 (15.3%) reported a history of alcohol consumption. Conclusion: The findings of this study suggest that environmental variables such as smoking and alcohol consumption, along with a family history of varicocele and demographic factors like age and occupation, may be associated with the prevalence of varicocele. However, the epidemiology of varicocele is not yet fully understood.
