fa مقایسه اثر قبل با بعد از تزریق داخل کورپوس کاورنوزوم Platelet-rich plasma در بهبود عملکرد جنسی مردان با اختلال نعوظ Comparison of the effect before and after intracorpus cavernosum Platelet-rich plasma injection in imporoving sexual function in men with erectile dysfunction پزشکی- اورولوژی Medicine - Urology پژوهشي اصیل Original Research زمینه و هدف: اختلال نعوظ (ED) مشکل رایجی در بین مردان است و استفاده از درمان پلاسمای غنی از پلاکت (PRP) برای درمان ED توجه فزاینده‌ای را به خود جلب کرده است، اما هنوز هیچ مدرک قطعی‌ای درباره اثربخشی آن وجود ندارد. هدف از انجام این مطالعه مقایسه اثر قبل با بعد از تزریق داخل کورپوس کاورنوزوم PRP در بهبود عملکرد جنسی مردان با اختلال نعوظ بوده است. مواد و روش‌ها: این مطالعه مداخله‌ای قبل و بعد تک‌بازویی بدون هیچ تصادف‌سازی است که روی ۳۲ نفر از مردان دارای اختلال نعوظ مراجعه‌کننده به بیمارستان کوثر سنندج انجام شد. در این مطالعه، ابتدا پس از تکمیل مشخصات دموگرافیک نمره اختلال عملکرد جنسی آن‌ها با استفاده از پرسش‌نامه IIEF اندازه‌گیری شد. سپس افراد PRP در روزهای صفر، بیست، چهل و شصت دریافت هربار پرسش‌نامه را پر کردند. ارزیابی شاخص‌های پیامد بیست روز بعد، چهل روز بعد و شصت روز بعد از دریافت PRP انجام شد. مهم‌ترین شاخص پیامد عملکرد جنسی بیماران است که با استفاده از پرسش‌نامه IIEF اندازه‌گیری شد. برای تجزیه‌وتحلیل داده‌ها، با توجه به کیفی‌بودن متغیر وابسته از آزمون‌های تی، کای اسکوئر و رگرسیون لجستیک استفاده شد. برای تمام آزمون‌ها سطح معناداری ۰/۰۵ در نظر گرفته شد. یافته‌ها: نتایج مطالعه نشان داد که میانگین سن بیماران ۵۷/۳۱ سال بوده است. نتایج نشان داد که میانگین امتیاز کلی قبل از تزریق۲/۶۳±۷/۴۳، بیست روز بعد تزریق ۳/۵۳±۹/۸۷، چهل روز بعد تزریق۳/۵۲±۱۰/۸۱ و میانگین امتیاز کلی شصت روز بعد تزریق ۳/۸۱ ± ۱۱/۴۶ بوده است. نتایج تحلیلی نشان داد بین امتیاز نعوظ کافی قبل و بعد از تزریق (بیست، چهل، شصت روز)، نعوظ کافی برای دخول، نعوظ کافی بعد از دخول قبل از تزریق و بیست روز بعد از تزریق تفاوت معنی‌دار وجود داشته است. همچنین، بین میانگین امتیاز کلی قبل از تزریق و بعد از تزریق (روز بیست، چهل و شصت) تفاوت معنی‌دار وجود داشته است و به نظر می‌رسد تزریق داخل کورپوس کاورنوزوم PRP در بازیابی عملکرد نعوظ طبیعی و میزان رضایت عملکرد جنسی افراد مورد مطالعه تأثیر بسزایی داشته است. (۰/۰۰۰۱P<)، اما بین امتیاز حفظ نعوظ تا کامل‌شدن رابطه قبل از تزریق و بعد از تزریق تفاوت معنی‌دار آماری مشاهده نشده است (۰/۰۵P>). نتیجه‌گیری: درمان PRP برای اختلال نعوظ اثربخشی امیدوارکننده‌ای را نشان داده است و قابلیت تبدیل‌شدن به گزینه درمانی جایگزین جدید را برای بیماران مبتلابه اختلال نعوظ دارد. Background and Aim: Erectile dysfunction (ED) is a common problem among males, and the use of platelet-rich plasma (PRP) therapy for the treatment of ED has attracted increasing attention; however, there is still no definitive evidence of its effectiveness. This study aimed to compare sexual function before and after intracavernosal platelet-rich plasma (PRP) injection in men with erectile dysfunction. Materials and methods: This single-arm, before-and-after interventional study without any randomization was conducted on 32 men with erectile dysfunction who were referred to Kowsar Hospital in Sanandaj, Iran. In this study, after completing demographic characteristics, their sexual dysfunction score was measured using the International Index of Erectile Function (IIEF) questionnaire. Then, the PRP subjects filled out the questionnaire on days 0, 20, 40, and 60 after receiving each PRP injection. Evaluation of outcome indicators 20 days later, 40 days later, and 60 days after receiving PRP was performed. The most important outcome indicator is the sexual function of the patients, which was measured using the IIEF. For data analysis, considering the qualitative nature of the dependent variable, t-test, chi-square, and logistic regression tests were used. For all tests, a significance level of 0.05 was considered. Results: According to the results, the mean age of the patients was 57.31 years. The results showed that the mean total score was 7.43 ± 2.63 before injection, 9.87 ± 3.53 at 20 days after injection, 10.81 ± 3.52 at 40 days after injection, and 11.46 ± 3.81 at 60 days after injection. The analytical results showed a significant difference between the score of sufficient erection before and after injection (days 20, 40, 60 ), sufficient erection for penetration, sufficient erection after penetration before injection, and 20 days after injection. There was also a significant difference between the mean total score before and after injection (days 20, 40, and 60), and it seems that intracorpus cavernosum PRP injection had a significant effect on restoring normal erectile function and satisfaction with sexual function of the subjects studied (P<0.0001). However, no statistically significant difference was observed in the score for maintaining an erection until completion of intercourse before and after the injection (P>0.05).
