fa اختصاصی سازی راهنمای جامع استانداردهای اعتبار بخشی ملی بیمارستان های ایران برای بیمارستان های تک تخصصی: یک مطالعه کمی کیفی مدیریت خدمات بهداشتی و درمانی Health Management Services پژوهشي اصیل Original Research <p dir="RTL" style="text-align:justify"><span style="font-size:11pt"><span style="font-family:Calibri,sans-serif"><strong><span style="font-size:12.0pt"><span b="" style="font-family:" zar="">زمینه و هدف: </span></span></strong><span style="font-size:12.0pt"><span b="" style="font-family:" zar="">در ایران و در سالیان اخیر، ملاک اعتباربخشی بیمارستان ها، نسل پنجم راهنمای جامع استانداردهای اعتبار بخشی ملی بوده است که برای همه انواع بیمارستان های عمومی و تک تخصصی به طور یکسان استفاده می گردد. مطابق با آخرین آمار منتشره وزارت بهداشت، از مجموع 1066 بیمارستان فعال در ایران، 14.2 درصد، بیمارستان</span></span><span dir="LTR" lang="FA" style="font-size:12.0pt"><span b="" style="font-family:" zar="">&lrm;</span></span><span style="font-size:12.0pt"><span b="" style="font-family:" zar="">های تک تخصصی هستند. هدف این پژوهش، اختصاصی سازی و انطباق راهنمای مذکور با شرایط و خصوصیات منحصر به فرد بیمارستان های تک تخصصی بود.</span></span></span></span></p> <p dir="RTL" style="text-align:justify"><span style="font-size:11pt"><span style="font-family:Calibri,sans-serif"><strong><span style="font-size:12.0pt"><span b="" style="font-family:" zar="">مواد و روش ها:</span></span></strong> <span style="font-size:12.0pt"><span b="" style="font-family:" zar="">این مطالعه آمیخته در دو فاز انجام شد: 1) فاز کیفی با 23 مصاحبه نیمه‌ ساختاریافته با ذینفعان کلیدی شامل مدیران داخلی و مدیران بهبود کیفیت بیمارستان های هدف، ارزیابان کشوری اعتباربخشی و کارشناسات مرکز نظارت و اعتباربخشی امور درمان وزارت بهداشت بود که با روش نمونه گیری هدفمند با حداکثر تنوع انتخاب شدند. ، و 2) فاز کمی با تکنیک دلفی تعدیل‌شده (2 دور) با مشارکت 28 متخصص.</span></span></span></span></p> <p dir="RTL" style="text-align:justify"><span style="font-size:11pt"><span style="font-family:Calibri,sans-serif"><strong><span style="font-size:12.0pt"><span b="" style="font-family:" zar="">یافته</span></span></strong><strong><span dir="LTR" lang="FA" style="font-size:12.0pt"><span b="" style="font-family:" zar="">&lrm;</span></span></strong><strong><span style="font-size:12.0pt"><span b="" style="font-family:" zar="">ها: </span></span></strong><span style="font-size:12.0pt"><span b="" style="font-family:" zar="">در مجموع 18 سنجه از راهنمای جامع اعتباربخشی، قابل ویرایش برای بیمارستان های تک تخصصی شناسایی گردیدند. از این تعداد، هفت سنجه مربوط به بیمارستان چشم پزشکی، چهار سنجه مربوط به بیمارستان سوختگی، دو سنجه مربوط به بیمارستان اطفال و پنج سنجه نیز مربوط به همه انواع بیمارستان های تک تخصصی بودند. </span></span></span></span></p> <p dir="RTL" style="text-align:justify"><span style="font-size:11pt"><span style="font-family:Calibri,sans-serif"><strong><span style="font-size:12.0pt"><span b="" style="font-family:" zar="">نتیجه‌گیری: </span></span></strong><span style="font-size:12.0pt"><span b="" style="font-family:" zar="">با توجه به شرایط و خصوصیات منحصربه ‌فرد بیمارستان‌های تک تخصصی ازجمله حضور پزشکان متخصص و به کارگیری تجهیزات پزشکی پیشرفته، می‌توان نسل پنجم راهنمای جامع استانداردهای اعتباربخشی ملی را برای انطباق با بیمارستان‌های مذکور بازنویسی کرد. پیامد اجرایی و سیاستی این عمل، تسهیل فرایند اعتباربخشی و کاهش هزینه های اعتباربخشی برای بیمارستان های تک تخصصی خواهد بود.</span></span></span></span></p> <p style="text-align:justify"><span style="font-size:11pt"><span style="font-family:Calibri,sans-serif"><strong><span style="font-size:12.0pt"><span new="" roman="" style="font-family:" times="">Background and Aim:</span></span></strong> <span style="font-size:12.0pt"><span new="" roman="" style="font-family:" times="">In Iran, in recent years, the criterion for hospital accreditation has been the fifth generation of the Comprehensive Guide to National Accreditation Standards, which is used equally for all types of hospitals regardless of size. According to the most recent statistics released by the Ministry of Health, of the 1,066 active hospitals in Iran, 14.2 percent are special hospitals. The aim of this research was to customize and adapt the aforementioned guide to the unique conditions and characteristics of special hospitals.</span></span></span></span></p> <p style="text-align:justify"><span style="font-size:11pt"><span style="font-family:Calibri,sans-serif"><strong><span style="font-size:12.0pt"><span new="" roman="" style="font-family:" times="">Materials and Methods: </span></span></strong><span style="font-size:12.0pt"><span new="" roman="" style="font-family:" times="">This mixed-method study was conducted in two phases: 1) a qualitative phase with 23 semi-structured interviews with key stakeholders including internal managers and quality improvement managers of target hospitals, national accreditation assessors, and experts from the Center for Monitoring and Accreditation of Medical Affairs of the Ministry of Health, who were selected using a purposive sampling method with maximum diversity, and 2) a quantitative phase with a modified Delphi technique (2 rounds) with the participation of 28 experts.</span></span></span></span></p> <p style="text-align:justify"><span style="font-size:11pt"><span style="font-family:Calibri,sans-serif"><strong><span style="font-size:12.0pt"><span new="" roman="" style="font-family:" times="">Results: </span></span></strong><span style="font-size:12.0pt"><span new="" roman="" style="font-family:" times="">A total of 18 issues from the Comprehensive Accreditation Guide were identified as editable for special hospitals. Among these, seven issues pertained to ophthalmology hospitals, four issues were associated with burn hospitals, two issues related to pediatric hospitals, and five metrics applied to all types of special hospitals.</span></span></span></span></p> <p style="text-align:justify"><span style="font-size:11pt"><span style="font-family:Calibri,sans-serif"><strong><span style="font-size:12.0pt"><span new="" roman="" style="font-family:" times="">Conclusion: </span></span></strong><span style="font-size:12.0pt"><span new="" roman="" style="font-family:" times="">Given the unique conditions and characteristics of special hospitals, including the presence of specialist physicians and the use of advanced medical equipment, the fifth edition of the Comprehensive Guide to National Accreditation Standards can be rewritten to adapt to these hospitals.
