fa کیفیت‌ زندگی‌ مرتبط‌ با سلامت‌ دهان در بیماران بی دندان: مقایسه ای بین اوردنچر متکی بر ایمپلنت و دست دندان کامل Oral Health-Related Quality of Life in edentulous Patients: A Comparison Between Implant-Supported Over Dentures and Complete Dentures دندانپزشکی Dentistry كاربردي Applicable زمینه و هدف: اگر چه از دست رفتن دندان ها با درجاتی از افت کیفیت زندگی همراه است اما متغیرهایی نظیر نوع پروتز می توانند، بر روی کیفیت زندگی اثر بگذارند. لذا هدف از مطالعه حاضر بررسی کیفیت زندگی مرتبط با سلامت دهان در بیماران درمان شده با دنچر کامل و اوردنچر متکی بر ایمپلنت بود. مواد و روش ها: در این مطالعه ی مقطعی توصیفی-تحلیلی ، 45 بیمار دارای دنچر کامل و 45 بیمار دارای اوردنچر متکی بر ایمپلنت که بین سال های 1399-1391 در بخش تخصصی پروتز دانشکده دندانپزشکی یزد درمان شده بودند، بصورت نمونه گیری آسان انتخاب و بررسی شدند. داده ها توسط پرسشنامه کیفیت زندگی مرتبط با سلامت دهان (14OHIP-) به انضمام متغیرهای دموگرافیک(سن، جنس، مدت زمان استفاده از پروتز، تعداد ایمپلنت و نوع اتچمنت) جمع آوری شدند. داده ها با استفاده از نرم افزار آماری  25-SPSS و آزمون های آماری من ویتنی، کروسکال والیس و ضریب همبستگی اسپیرمن تجزیه و تحلیل شدند. یافته ها: میانه ی  نمره کیفیت زندگی مرتبط با سلامت دهان در بیماران دارای دنچرکامل 7 و در بیماران دارای اوردنچر متکی بر ایمپلنت 1 بودکه این تفاوت از نظر آماری معنی دار بود(001/0>P-value). تفاوت آماری معنی داری در حیطه های ناراحتی فیزیکی، ناراحتی روانی و ناتوانی فیزیکی در دو گروه بیماران مشاهده شد (05/0>P-value). تفاوت آماری معنی داری در میانه ی نمره کیفیت زندگی مرتبط با سلامت دهان دو گروه بیماران بر حسب سن، جنس، مدت زمان استفاده از پروتز، تعداد ایمپلنت و نوع اتچمنت وجود نداشت (05/0<P-value). نتیجه گیری: بیماران دارای اوردنچر متکی بر ایمپلنت نسبت به افراد دارای دست دندان کامل از کیفیت زندگی وابسته به سلامت دهان بالاتری برخوردار بودند بخصوص در حیطه های ناراحتی فیزیکی، ناراحتی روانی و ناتوانی فیزیکی. کلید واژه ها: سلامت دهان، کیفیت زندگی، دندان مصنوعی، پروتز دندان، متکی بر ایمپلنت   Background and Aim: Although tooth loss is associated with varying degrees of reduced quality of life, factors such as the type of prosthesis can affect the quality of life. Therefore, the aim of this study was to evaluate the oral health related quality of life in patients treated with complete dentures and implant-supported over dentures. Materials and Methods: In this cross-sectional descriptive- analytical study,45 patients treated with complete dentures and 45 patients with implant-supported over dentures who were treated at the prosthetics department of Yazd Dental School between 2012-2020, were selected through convenience sampling and analyzed. Data were collected using the Oral Health Impact Profile (OHIP-14) questionnaire along with demographic variables (age, gender, duration of prosthesis use, number of implants and type of attachment). The data were analyzed using SPSS software version 25 and statistical tests including Mann-Whitney, Kruskal-Wallis, and Spearman's correlation coefficient. Results: The median score of oral health related quality of life in patients with complete prosthetic was 7 and in patients with implant-supported over denture was 1, this difference was statistically significant (P-value < 0.001).There were statistically significant differences in the physical discomfort, psychological discomfort, and physical disability domains between the two groups of patients (P-value <0.05). There was no statistically significant difference in the median scores of oral health related quality of life of the two groups of patients based on age, gender, duration of prosthesis use, number of implants and type of attachment (P-value <0.05).
