fa سیمای جدید اپیدمیولوژی بیماری لیشمانیوز پوستی در یک کانون آندمیک در استان گلستان New Epidemiological Feature of Cutaneous Leishmaniasis in an Endemic Focus in Golestan Province انگل شناسی و قارچ شناسی Parasitology and Mycology پژوهشي اصیل Original Research زمینه و هدف: لیشمانیوزپوستی (CL)، به عنوان یک مشکل بهداشتی مهم در ایران و یکی از بیماری‌های فراموش شده در جهان است که توسط گونه های لیشمانیا ایجاد می‌گردد. هدف از این مطالعه بررسی اپیدمیولوژی بیماری لیشمانیوز پوستی و فاکتورهای خطر مرتبط با آن در یک کانون آندمیک در استان گلستان هست. مواد و روش ها: مطالعه حاضر از نوع مقطعی – تحلیلی بوده و بر روی بیماران مشکوک به لیشمانیوز پوستی مراجعه کننده به مرکز لیشمانیوز پوستی شهرستان گنبد کاووس در طی سال‌های 1397 و 1398 انجام گرفت و تمام اطلاعات دموگرافیک و اپیدمیولوژیک بیماران ثبت گردید. حضور انگل لیشمانیا با تهیه گسترش مستقیم از ضایعات پوستی و رنگ آمیزی گیمسا انجام گرفت. همچنین از روش PCR جهت تعیین گونه انگل استفاده شد. یافته‌ها: از 1979 ضایعه مشکوک به لیشمانیوز پوستی، 1704 (87/4 درصد) مورد مثبت بودند. از نظر جنسیت، 898 (52/7 درصد) نفر از مبتلایان مرد و 806 (47/3 درصد) نفر زن بودند. بیشترین موارد ابتلا درگروه‌های سنی کمتر از 5 سال ( 25/93درصد) و 45- 25 سال ( 25/41درصد)  مشاهده شد. در این مطالعه با استفاده از روش PCR-ITS، گونه انگلهای لیشمانیا در تمام نمونه های مورد بررسی، لیشمانیا ماژور بود. همچنین سه مورد کریتیدیا فاسیکولاتا در بیماران گزارش گردید. نتیجه‌گیری: این بررسی اطلاعات ارزشمندی در مورد جنبه های اپیدمیولوژیک لیشمانیوز پوستی در یک کانون بومی قدیمی در شمال ایران ارائه داده است که می تواند پایه ای برای استراتژی‌های کنترل بیماری در آینده باشد. Background and Aim: Cutaneous leishmaniasis (CL) is an important health problem in Iran and one of the world ̛s neglected diseases which is caused by Leishmania spp. The aim of this study was to investigate the epidemiology of cutaneous leishmaniasis and its associated risk factors in an endemic center in Golestan Province. Materials and Methods: This descriptive cross-sectional study included patients with suspected CL who had been referring to the health centers of Gonbad–e Qabus town, between 2018 and 2020. Demographic and epidemiological data of CL patients were recorded and diagnosis of Leishmania spp. was made by Giemsa staining of direct smear. PCR was also used to determine and confirm the parasite species. Results: From 1979 suspected CL lesions, 1704(87.4%) had positive smears. 898 (52.7%) patients were male and 806 (47.3%) were female. The highest incidence rate was observed in the patients of less than 5 (25.93%) and those between 25 and 45 years of age (25.41%). In all samples in this study, Leishmania major was the only Leishmania spp detected by ITS-PCR. Also, Critidia fasciulata was identified in three patients.
