بررسی هیپرکلسیوری و علائم همراه آن در دانشآموزان دبستانی شهر سنندج

خلاصه
زمینه و هدف: هیپرکلسیوری ایدیوپاتیک یک اختلال ارثی متابولیک است. این اختلال متابولیک با عوارض و پیامدهای زیادی همراه است. از جمله تأثیر هیپرکلسیوری بر کاهش تراکم استخوان میباشد. این مطالعه با هدف تعیین شیوع هیپرکلسیوری ایدیوپاتیک در کودکان دبستانی سنندج انجام شد.
روش بررسی: این مطالعه یک مطالعه توصیفی- تحلیلی است که 506 نفر از دانشآموزان ابتدائی شهر سنندج در سال 1384 که با روش نمونهگیری تصادفی خوشهای انتخاب شده بودند، مورد مطالعه قرار گرفتند. کسانی که نسبت Ca/Cr در نمونه ادرار صبحگاهی بیش از 2/0 داشتند به عنوان تست مثبت برای غربالگری در نظر گرفته شدند. 32 نفر در مرحله دوم مورد آزمایش قرار گرفتند. کسانی که در ادرار 24 ساعته دفع ادراری کلسیم بیش از mg/Kg 4 داشتند به عنوان هیپرکلسیوریک در نظر گرفته شدند که شامل 3 پسر و 12 دختر بودند. دادههای گرد آوری شده وارد نرم افزار آماری SPSS win شده و با استفاده از آمار توصیفی به صورت جداول توزیع فراوانی ارائه شد. همچنین جهت تحلیل دادهها از تستهای آماری T test و X2 استفاده شد.
یافتهها: کودکان شامل 248 دختر و 258 پسر بودند. آنهائیکه نسبت کلسیم به کراتینین ادرار بالاتر از 2/0 داشتند شامل 32 نفر که شامل 11 پسر و 21 دختر بودند که از میان اینها 15 نفر شامل 3 پسر و 12 دختر دفع کلسیم ادراری بیش از mg/Kg/day 4 داشتند که شامل 3 درصد کل دانشآموزان (2/1درصد پسران و 8/4درصد) دختران بودند.
نتیجهگیری: نتایج این مطالعه نشان داد که شیوع هیپرکلسیوری در دختران بیشتر از پسران بود. با توجه به تأثیر هیپرکلسییوری بر تراکم استخوان و رشد استخوانی که بخصوص در دختران اهمیت بیشتری دارد لازم است این موضوع در یک مطالعه جامعتر مورد بررسی قرارگیرد.

ABSTRACT
Background and Aim: Determination of prevalence rate of idiopathic hypercalciuria(IH) in primary school students in Sanandaj.
Material and Methods: This was a descriptive-analytic study and included 506 primary school students. First we measured the ratio of urine calcium to urine creatinine (UCa/UCr) as screening test then 24 hour urine calcium of the patients who had UCa/UCr ratio more than 0.2 mg/mg (31patients) was measured. Those with 24 hours urine calcium of more than 4 mg/kg were regarded hypercalciuric, (3 boys and 12 girls). Data were introduced into SPSS win soft ware, represented in the form of frequency distribution table and analyzed by means of X2 and T-tests.
Results: 248 patients were female and 258 were male. 32 patients (6.32%), 11 boys and 21 girls had UCa/UCr ratio more than 0.2 mg/mg, but 15 patients (3 boys and 12 girls) had excretion of more than 4mg/kg/day calcium in urine which encompassed 3% of the students (1.2% boys, 4.8% girls).
Conclusion: The results of this study indicated the Incidence of hypercalciuria among student girls was more than that of boys. 95th percentile for urine Ca/Cr is 0.198 for boys and 0.253 for girls. Considering the effect of hypercalciuria on bone density and growth which has more importance in girls, it is recommended to evaluate such effect in a more comprehensive study.
