fa کیفیت مستندسازی پرونده های بیماران توسط دانشجویان پزشکی و پزشکان در بیمارستان توحید سنندج مدیریت خدمات بهداشتی و درمانی Health Management Services پژوهشي اصیل Original Research <strong><span style="font-family:B Zar;">زمینه و هدف:</span></strong><span style="font-family:B Zar;"> مطالعات قبلی در ایران نشان داده است کیفیت مستندسازی پرونده&shy;های پزشکی توسط پزشکان و کادر درمانی چندان رضایت بخش نیست. </span><span style="font-family:B Zar;">هدف از انجام مطالعه حاضر بررسی </span><span style="font-family:B Zar;">وضعیت ثبت پرونده&shy;های پزشکی و در بیمارستان توحید سنندج بود. </span><span style="font-family:B Zar;"></span><br> <strong><span style="font-family:B Zar;">مواد و روش&shy;ها: </span></strong><span style="color:black;"><span style="font-family:B Zar;">در یک مطالعه توصیفی تحلیلی، پرونده</span></span> <span style="font-family:B Zar;">پزشکی 430 نفر از </span><span style="font-family:B Zar;">بیماران بستری در سرویس&shy;های مختلف بیمارستان توحید سنندج در شش ماهه اول سال 1396 بررسی شد<strong>.</strong></span><span style="font-family:B Zar;"> متغیرهای مورد نیاز بر اساس دستورالعمل کشوری مستندسازی پرونده پزشکی و با استفاده از یک چک لیست جمع&shy;آوری گردید. </span><span style="font-family:B Zar;">داده&shy;ها با استفاده از نرم افزار</span><span dir="LTR"><span style="font-size:11.0pt;">SPSS V.20 </span></span>&nbsp;<span style="font-family:B Zar;">و آزمون مجذور کای آنالیز شد. </span><strong><span style="font-family:B Zar;"></span></strong><br> <strong><span style="font-family:B Zar;">یافته&shy;ها: </span></strong><span style="font-family:B Zar;">در مطالعه حاضر تشخیص صحیح منطبق بر اصول تشخیص نویسی در 261 مورد (</span><span style="font-family:B Zar;">60/7%</span><span style="font-family:B Zar;">) از فرم&shy;های پذیرش رعایت شده بود و ساعت اولین ویزیت پزشک در 282 مورد (</span><span style="font-family:B Zar;">66%</span><span style="font-family:B Zar;">) از پرونده&shy;ها ثبت شده بود. <span style="color:black;">بیش از نیمی از برگه &shy;های شرح حال و اکثر گزارش&shy;های اولیه، روزانه و انتهایی توسط دانشجویان پزشکی تکمیل نشده و یا ناقص تکمیل شده بود. اساتید به ترتیب 133 مورد (40/7%)، 76 مورد (22%) و 5 مورد (7/2%) از شرح حال های نوشته شده توسط دستیاران، کارورزان و کارآموزان را مورد ارزیابی قرار داده بودند. </span>563 مورد (66/2<span style="color:black;">%</span>) از دستورات پزشک در پرونده&shy;ها به&shy;طور صحیح ثبت شده بودند. میزان ثبت صحیح دستورات پزشکی توسط کارورزان، دستیاران و اساتید به ترتیب 101 مورد (69/2<span style="color:black;">%</span>) ، 186 مورد &nbsp;(87/7<span style="color:black;">%</span>) و 240 مورد (53/7<span style="color:black;">%</span>) بود. </span><br> <strong><span style="font-family:B Zar;">نتیجه&shy;گیری: </span></strong><span style="color:black;"><span style="font-family:B Zar;">بر اساس یافته&shy;های این مطالعه، برگزاری دوره&shy;های آموزشی مستند&shy;سازی صحیح پرونده&shy;های درمانی برای پزشکان و دانشجویان این رشته و نظارت دقیق تر اساتید و پزشکان معالج بر عملکرد دانشجویان </span></span><span style="font-family:B Zar;">در </span><span style="color:black;"><span style="font-family:B Zar;">بیمارستان&shy;های آموزشی دانشگاه علوم پزشکی کردستان ضروری به نظر می&shy;رسد. </span></span> <strong>Background and Aim:</strong> Previous studies in Iran have shown that the quality of documentation of medical records by physicians and medical staff is not very satisfactory. The purpose of this study was to investigate the quality of medical records documentation in Tohid teaching hospital, , in Sanandaj, Iran.<br> <strong>Materials and Methods:</strong> In this descriptive-analytical study, medical records of 430 patients admitted to various services of Tohid Hospital in Sanandaj during the first six months of 2017 were examined. Variables were collected using a checklist based on the national regulations for the documentation of medical records. Data were analyzed using SPSS V.20 software and Chi-square test.<br> <strong>Results</strong>: In the present study we observed registration of the correct diagnosis, in accordance with the principles of diagnosis, in 261 (60.7%) of admission forms and the time of first visit of the physician had been recorded in 282 (66.0%) cases. More than half of the history sheets and most of the initial, daily, and final reports were incomplete. The professors had evaluated 133 (40.7%), 76 (22.0%) and 5 (7.2%) history sheets completed by the residents, interns and externs respectively. About 563 (66.2%) cases of the doctor&#39;s prescriptions had been properly recorded in the files.
