fa بررسی کلونالیتی سویه‌های انتروکوکوس فکالیس و انتروکوکوس فیسیوم مقاوم به مقادیر بالای جنتا مایسین جداشده از فاضلابهای شهر تهران عمومى General پژوهشي اصیل Original Research <p> <strong> چکیده </strong></p><p> <strong><i> زمینه و هدف </i></strong><strong>: </strong>در سالهای اخیر پیدایش انتروکوک‌های مقاوم به مقادیر بالای پنی‌سیلین، گلیکو پپتیدها و جنتامایسین به تنهایی یا بصورت چند مقاومتی باعث بروز مشکلات حاد در درمان عفونتهای انتروکوکی شده است. انتروکوک‌ها جزء فلور نرمال روده هستند و از این راه به فاضلابها راه پیدا می‌کنند. یکی از روشهای بررسی جمعیت انتروکوکی غربالگری فاضلابها ازحیث وجود انتروکوک‌ها می‌باشد. هدف تحقیق حاضر بررسی کلونالیتی در بین سویه‌های HLGR انتروککی جدا شده از فاضلاب در تهران می‌باشد . </p><p> <strong><i> روش بررسی </i></strong><strong>: </strong>تعداد 140 ایزوله از سه تصفیه خانه فاضلاب در مناطق مختلف شهر تهران در محدوده زمانی آذر ماه 85 تا اردیبهشت ماه 86 جمع‌آوری شد. تعیین جنس و گونه ایزوله‌ها و ژنهای مقاومت به جنتامایسین توسط روش PCR و تست حساسیت آنتی‌بیوتیکی و MIC نیز با استفاده از استاندارد CLSI به انجام رسیده و روش Biochemical fingerprinting (PhP typing) با هدف تایپینگ سویه‌های HLGR انتروکوکی بکار رفت. </p><p> <strong><i> یافته‌ها </i></strong><strong>: </strong>انتروکوکوس فکالیس 26% و انتروکوکوس فیسیوم 74%، شایعترین گونه‌های جدا شده از نمونه‌های فاضلاب بودند. بالاترین مقاومت آنتی‌بیوتیکی در بین سویه‌های فیسیوم نسبت به اریترومایسین و در بین سویه‌های فکالیس نسبت به تتراسیکلین دیده شد. MIC تمام سویه‌های HLGR انتروکوکی g/ml μ 1024 ≤ بود و اکثریت آنها دارای ژن aac (6')-Ie-aph (2'')-Ia بودند و فقط یک سویه انتروکوکوس فیسیوم دارای ژن aph(2")-Ic بود. روش PhP typing در گونه فکالیس 16 تایپ مختلف را با 910/0 Di= و درگونه فیسیوم با 945/0 Di= ، 50 تایپ مختلف را مشخص نمود. </p><p> <strong><i> نتیجه‌گیری </i></strong><strong>: </strong>حضور ژن aac (6')-Ie-aph (2'')-Ia در اکثریت ایزوله‌های HLGR ، نشان دهنده انتشار وسیع این ژن در جمعیت انتروکوکی فاضلاب است. شیوع متنوع کلونال در جمعیت انتروکوک‌های HLGR و چند مقاومتی در فاضلاب تهران نشان دهنده نقش احتمالی فاضلابها در چرخش این سویه‌ها در جامعه می‌باشند. </p> <p> <strong> ABSTRACT </strong></p><p> <strong><i> Background and Aim: </i></strong>The development of high-level resistance to aminoglycosides, penicillin and glycopeptides singly and in combination by enterococci, has important clinical implications. Enterococci are normal flora of intestine and are released into the environment through faeces. One of the investigation methods on enterococcal population is the screening of sewage treatment plants for presence of enterococci. The aim of this study was to determine clonal diversity among HLGR enterococcal isolates , obtained from the sewage treatment plants (STPs) in Tehran. </p><p> <strong><i> Material and Methods: </i></strong>From November 2005 to May 2006 a total of 140 enterococcal isolates were collected from 3 sewage treatment plants (STP) located at different parts of Tehran. Confirmation of species and detection of gentamicin resistance genes were performed by PCR method. Antimicrobial susceptibility and MIC determination tests were performed on the basis of CLSI standard. Php typing was used for typing of enterococcal HLGR isolates. </p><p> <strong><i> Results: </i></strong>E. faecalis (26%) and E. faecium (74%) were the most common species in STPs samples. E. faecium had the highest rate of resistance to tetracycline. The MICs for all of enterococcal HLGR isolates were ≥1024 µg/ml and most enterococci contained aac (6')-Ie-aph (2'')-Ia gene. Only one E. faecium isolate showed aph (2")-Ic gene. Php typing of sewage HLGR E. faecalis and E. faecium isolates yielded 16 and 50 patterns with a diversity index of 0.910 and 0.945, respectively. </p><p> <strong><i> Conclusion: </i></strong>The presence of aac (6')-Ie-aph (2'')-Ia gene in most of HLGR isolates, suggested widespread dissemination of this gene in the enterococcal isolates in sewage.
