تأثیر ساکشن بسته ترشحات ریوی بر پنومونی وابسته به ونتیلاتور

چکیده
زمینه و هدف: پنومونی وابسته به ونتیلاتور از عفونتهای شایع بیمارستانی در بخشهای مراقبت ویژه است که با مرگ و میر بالائی همراه است. ساکشن داخل تراشه، روشی است که بطور معمول جهت پاکسازی ترشحات راههای هوائی در بیماران تحت ونتیلاتور مورد استفاده قرار میگیرد. تأثیر روشهای ساکشن باز و بسته بر بروز پنومونی بدرستی مشخص نمیباشد. این مطالعه با هدف مقایسه ساکشن به روش باز و بسته بر میزان بروز پنومونی در بیماران تحت تهویه مکانیکی انجام گرفت.
روش بررسی: این مطالعه یک کارآزمائی بالینی بود که بر روی 156 بیمار بستری در بخشهای مراقبت ویژه بیمارستانهای آموزشی وابسته به دانشگاه علوم پزشکی اراک انجام گرفت. بیمارانی که کمتر از 24 ساعت بعد از بستری شدن در بیمارستان، به بخش ویژه وارد میشدند و از زمان لولهگذاری داخل تراشه، کمتر از 6 ساعت گذشته بود و به تهویه مکانیکی نیاز داشتند بطور تصادفی به دو گروه تقسیم شدند. بیماران گروه کنترل (74 نفر) با روش باز و به روش معمول، و بیماران گروه مداخله (82 نفر) با روش بسته ساکشن شدند. پس از گذشت 72 ساعت تمام بیماران با استفاده از مقیاس پنومونی باکتریال که یک ابزار استاندارد است مورد بررسی قرار گرفتند.
یافتهها: نتایج نشان داد که میزان بروز پنومونی در گروه مداخله (روش بسته) 28% و در گروه کنترل (روش باز) 6/48% میباشد. با استفاده از آزمون کای اسکویر نشان داده شد که اختلاف معنیداری بین دو گروه از نظر میزان بروز پنومونی وجود دارد (01/0=p). بروز پنومونی در بیمارانی که سابقه مصرف سیگار داشتند بیشتر بود، اما بین زن و مرد اختلافی دیده نشد.
نتیجهگیری: با توجه به این که در ساکشن بسته خطر انتقال آلودگی از طریق وسایل و دستهای پرسنل کمتر میباشد، این روش در مقایسه با ساکشن باز میتواند منجر به کاهش خطر ابتلا به پنومونی وابسته به ونتیلاتور گردد.

ABSTRACT
Background and aim: Ventilator-associated pneumonia (VAP) is one of the common nosocominal infections in intensive care units (ICU) which leads to high mortality rates. Endotracheal suctioning is routinely performed in mechanically ventilated patients to clear secretions. The aim of this study was to compare the effect of closed versus open endotracheal suction methods on development of ventilator-associated pneumonia.
Materials and Methods: This randomized control trial included 156 hospitalized patients in intensive care units of the hospitals of Arak University of Medical Sciences. Patients who had required mechanical ventilation and been transferred to ICU in less than 24 hours after their admission and had endotracheal tubes for less than six hours were assigned randomly into two groups. Routine open suction and closed suction methods were used for control (n=74) and intervention (n=82) groups respectively. After 72 hours the patients were examined for signs and symptoms of pneumonia based on clinical pulmonary infection score (CPIS).
Results: The results of this study showed the incidence of pneumonia in the intervention and control groups were 28% and 48.6% respectively. Considering the incidence of pneumonia, there was a significant difference between the two groups (P=0.01). Patients with history of cigarette smoking had increased incidence of pneumonia but no difference was detected in the incidence of pneumonia between males and females.
Conclusion: Considering the decreased risk of transfer of contamination via medical devices and personnel's hands, use of closed suction method can lead to a decrease in the incidence of ventilator associated pneumonia.
