fa تحلیل مفهوم تفکر انتقادی در پرستاری بالینی عمومى General پژوهشي اصیل Original Research <p dir="RTL" style="margin-bottom:0in;margin-bottom:.0001pt;text-align:justify;line-height:normal;"><strong><span style="font-family:b zar;"><span style="font-size:12.0pt;">مقدمه:</span></span></strong><span style="font-family:b zar;"><span style="font-size:12.0pt;"> پیر شدن جمعیت، افزایش تفاوت های فرهنگی- اجتماعی در جوامع، افزایش بیماریهای مزمن، کاهش منابع مالی، تغییرات برق آسای تکنولوژی، تأکید بر ارتقاء سلامت و مراقبت در منزل، افزایش به نیاز به مراقبت های بیمار محور و عملکرد مبتنی بر شواهد ، محیط های بهداشتی- درمانی را بیش از پیش پیچیده ساخته اند.در چنین محیط هایی فراهم آوردن مراقبت های ایمن و اثر بخش از بیماران که منطبق بر معیارها و استانداردها باشد، تنها با توسعه ی مهارتهای تفکر انتقادی در پرستاران امکان پذیر خواهد بود. در پرستاری تعاریف متعددی در خصوص مفهوم تفکر انتقادی وجود دارد که اغلب آنها عمومی بوده و گاه فاقد همبستگی با عملکردهای بالینی و حرفه ای پرستار می باشد.</span></span></p> <p dir="RTL" style="margin-bottom:0in;margin-bottom:.0001pt;text-align:justify;line-height:normal;"><strong><span style="font-family:b zar;"><span style="font-size:12.0pt;">روش بررسی:</span></span></strong> <span style="font-family:b zar;"><span style="font-size:12.0pt;">جهت دستیابی به یک تعریف عملیاتی در خصوص تفکر انتقادی در پرستاری بالینی، از استراتژی تحلیل مفهوم هشت مرحله ای &quot;راجرز&quot; جهت تعیین پیشایندها ، ویژگی ها و پسامدها استفاده گردید. جستجو در پایگاه های داده ای معتبر در فاصله ی سالهای 1995 تا 2014 برای دستیابی به منابع انگلیسی و فارسی منجر به آنالیز 58 مقاله، 3 پایان نامه و دو فصل از دو کتاب بر اساس مدل استقرایی&quot; راجرز &quot; گردید .</span></span></p> <p dir="RTL" style="margin-bottom:0in;margin-bottom:.0001pt;text-align:justify;line-height:normal;"><strong><span style="font-family:b zar;"><span style="font-size:12.0pt;">یافته ها:</span></span></strong><span style="font-family:b zar;"><span style="font-size:12.0pt;"> نتایج حاصل از آنالیز داده ها بیانگر آن بود که تفکر انتقادی در پرستاری بالینی دارای3 پیشایند ، 6 ویژگی شامل به کارگیری فرآیند پرستاری در برخورد با موقعیت های بالینی، عملکرد کلی نگر</span></span> <span style="font-family:b zar;"><span style="font-size:12.0pt;">و جامع، به کارگیری مهارتهای هوش هیجانی در برخورد با موقعیت ها، استفاده از الگوهای مختلف جهت شناخت، عوامل زمینه ساز تفکر انتقادی و عناصر و اجزا تفکر انتقادی می باشد. دستیابی به مهارتهای تفکر انتقادی منجر به پیامدهای مثبتی نظیر مراقبت ها پرستاری بیمار محور و موثر، خلاقیت، عملکرد مبتنی بر شواهد و حرفه ای شدن در پرستاری خواهد گردید.</span></span></p> <p dir="RTL" style="margin-bottom:0in;margin-bottom:.0001pt;text-align:justify;line-height:normal;"><strong><span style="font-family:b zar;"><span style="font-size:12.0pt;">نتیجه گیری: </span></span></strong><span style="font-family:b zar;"><span style="font-size:12.0pt;">یافته های این مطالعه منجر به دستیابی به یک تعریف قابل درک و شفاف در خصوص مفهوم تفکر انتقادی در پرستاری بالینی گردید. این تعریف خواهد توانست بسیاری از تفاسیر و استفاده های نادرست از این مفهوم را در حرفه ی پرستاری اصلاح نموده، و با ایجاد یک ساختارو چارچوب صحیح جهت ارائه ی عملکردهای بالینی اثر بخش ، متضمن استقرار شایستگی حرفه ای درپرستاری گردد.</span></span></p> <p dir="RTL" style="margin-bottom:0in;margin-bottom:.0001pt;text-align:justify;line-height:normal;"><strong><span style="font-family:b zar;"><span style="font-size:12.0pt;">کلید واژه:</span></span></strong><span style="font-family:b zar;"><span style="font-size:12.0pt;"> تفکر انتقادی، آنالیز مفهوم، پرستاری بالینی، ایران</span></span></p> <p dir="RTL" style="margin-bottom:0in;margin-bottom:.0001pt;text-align:justify;"><span style="font-family:b zar;"><span style="font-size:13.0pt;">وصول مقاله:23/8/95 اصلاحیه نهایی:4/11/95 پذیرش:18/11/95&nbsp;</span></span></p> <p><strong>Ba</strong><strong>ckground and Aim: </strong>Aging of population, increased cultural and social differences in societies, increased rate of chronic diseases, reduction of financial resources, the flash-like technological changes, emphasis on home care, increased &nbsp;trend toward patient-centered care and evidence based practice have resulted in more complexity in clinical environments. Providing standard safe and effective care in such environmental conditions can only be achieved by development of critical thinking skills in the nursing staff. There are many definitions about critical thinking in nursing. Many of these explanations are complicated, without significant relation to the clinical and professional performances.</p> <p><strong>Material and M</strong><strong>e</strong><strong>t</strong><strong>hods:</strong> We used eight step Rodgers&rsquo; concept analysis method to provide a clear definition about critical thinking in clinical nursing to determine antecedents, attributes and consequences. We searched valid online data bases for in English and Persian articles from 1995 to 2014. Finally, 58 articles 3 dissertations and 2 chapters from two books were analyzed by means of Rodger&#39;s inductive approach.</p> <p><strong>Results:</strong> The results of this study showed that, critical thinking in clinical nursing have 3 antecedents and 6 attributes including Use of nursing process in clinical situation, holistic and comprehensive performance<strong><span dir="RTL">،</span></strong> Use of different patterns in order to achieve understanding, Using emotional intelligence skills in facing with clinical situations, facing with factors for critical thinking and elements of critical thinking. Achievement of critical thinking leads to positive outcomes such as effective and patient-centered care, creativity, evidence based practice and professionalism.</p> <p><strong>Conclusions:</strong> The result of this study let to development of a clear and comprehensive definition of critical thinking in clinical nursing. This definition can correct many misconceptions and misuses of the concept of critical thinking. The findings also can help clinical nurses to create a framework and guidelines for effective clinical performance.
