fa بررسی شیوع ژن مقاومت بتالاکتامازی CTX-M-2 در اشریشیاکلی جدا شده از عفونت ادراری در شهرستان سنندج Prevalence of (CTX-M-2) beta lactamase gene in E.coli isolated from patients with urinary tract infections (UTI) in Sanandaj, Iran عمومى General پژوهشي اصیل Original Research زمینه و هدف: در سال های اخیر، تولید آنزیم های بتالاکتامازی با طیف وسیع (ESBLs) در میان ایزوله های بالینی به ویژه باکتری اشریشیاکلی شیوع فراوانی یافته است. تولید آنزیم بتالاکتاماز در باکتری اشریشیاکلی مشکلات فراوانی را در درمان ایجاد نموده است. ژن CTX-M-2 یکی از چندین عوامل مولد مقاومت های ناشی از  بتالاکتامازهای وسیع الطیف می باشد. هدف از این مطالعه بررسی الگوی حساسیت آنتی بیوتیکی نسبت به آنتی بیوتیک های بتالاکتام و بررسی میزان ژن  CTX-M-2در نمونه ادراری اشرشیاکلی بود. روش بررسی: در این مطالعه 260 نمونه ادراری از مراکز درمانی شهرستان سنندج جمع آوری شد و 100 ایزوله E.coli  توسط آزمایش های بیوشیمیایی تأیید گردید. در مرحله بعد تست تعیین حساسیت نسبت به 11 آنتی بیوتیک با روش Diffusion  Disk منتخب انجام شد و سپس طبق دستور العمل CLSI با روش Combined Disk ، اشریشیاکلی های تولید کننده ESBLs در بین آنها مورد شناسایی قرار گرفت. در نهایت سویه های ESBLs مثبت ، توسط روش PCR ، از نظر ژن  CTX-M-2مورد بررسی قرار گرفتند. یافته ها: نتایج حاصل از تست های فتوتیپی نشان داد که از کل 100 سویه اشریشیاکلی تعداد 27 (27%) سویه تولید کننده ESBLs   بودند. طی روش PCR نیز مشخص شد که از این میان تعداد 2 (40/7) سویه تولید کننده CTX-M-2 هستند. نتیجه گیری: با توجه به درصد بالای مقاومت به سفالوسپورین های نسل سوم، انجام دقیق آزمایش های آنتی بیوگرام قبل از تجویز آنتی بیوتیک در عفونت های ناشی از ارگانیسم های تولید کننده ESBLs، یک ضرورت اجتناب ناپذیر است. کلید واژه: اشریشیاکلی، بتالاکتامازهای وسیع الطیف،  CTX-M-2. <span dir="RTL" lang="FA" style="font-family: "B Zar"; font-size: 12pt; mso-bidi-language: FA; mso-fareast-font-family: "Times New Roman"; mso-ansi-language: EN-US; mso-fareast-language: EN-US; mso-ascii-font-family: "Times New Roman"; mso-hansi-font-family: "Times New Roman";">وصول مقاله:25/5/94 اصلاحیه نهایی:3/8/94 پذیرش:17/8/94 Background and Aim: In recent years, the production of extended-spectrum beta-lactamases (ESBLs) among clinical isolates of bacteria in particular E.coli has been on the rise. Production of beta-Lactamase in E.coli has caused many problems in the treatment of the patients. The CTX-M-2 gene is one of the several factors producing resistance due to ESBL. The aim of this study was to evaluate the antimicrobial susceptibility to beta-lactam antibiotics and investigation of the CTX-M-2 gene level in E. coli isolated from urine samples. Material and Methods: In this study, 260 UTI samples were collected from medical centers in Sanandaj and 100 E.coli isolates were collected and confirmed by biochemical tests. Then susceptibility test to 11 selected antibiotics were performed by disk diffusion method and the ESBLs producing strains were identified by the combined disk method. Using PCR method, ESBLs positive strains were examined for the presence of CTX-M-2 gene. Results: The results of the phenotype tests showed that out of 100 E.coli strains, 27 (27%) were ESBLs producing. Also, PCR showed that among these 27 strains, 2 (7.4%) strains contained CTX-M-2 gene. Conclusion: Considering the high rate of resistance to the third generation cephalosporins, careful antibiogram tests is an inevitable necessity before prescribing antibiotics for the treatment of infections caused by ESBLs producing organisms.
