مقایسه اثرات آرام بخشی پروپوفول-کتامین با پروپوفول هنگام انجام بلوک رتروبولبار در جراحی کاتاراکت

زمینه و هدف: بلوک رتروبولبار یک فرآیند نسبتاً دردناک و نامطلوب برای بیمار به شمار میرود، لذا هنگام انجام بلوک جهت ایجاد راحتی و ایمنی بیمار از داروهای آرام بخش استفاده شود. این مطالعه با هدف شناخت اثرات آرام بخشی کتامین و پروپوفول نسبت به اثرات آرام بخشی پروپوفول در بیمارانی که با بلوک رتروبولبار تحت عمل جراحی کاتاراکت قرار میگیرند، انجام شده است.
روش بررسی: در این مطالعه، 60 بیمار تحت عمل جراحی کاتاراکت با بلوک رتروبولبار به صورت تصادفی در دو گروه، قرار گرفتند. تحت شرایط یکسان، در گروه پروپوفول بیماران جهت انجام آرام بخشی، mg/kg3/0 پروپوفول و در گروه کتامین به همراه پروپوفول بیماران mg/kg 15/0 کتامین بعلاوهmg/kg 3/0 پروپوفول دریافت کردند. علایم حیاتی، وضعیت آرام بخشی و خوابآوری و فشار داخل چشم، فراموشی، زمان شروع و مدت زمان آرام بخشی در تمام بیماران ثبت شد. همچنین عوارض جانبی در عرض 24 ساعت بعد از عمل را بررسی نمودیم. دادههای کمی بوسیله t-test و دادههای کیفی توسط آزمون X2 تحلیل گردید.
یافتهها: مدت زمان آرام بخشی بیمار در گروه پروپوفول-کتامین (8/2±8/5 دقیقه) طولانیتر از گروه پروپوفول (4/2±1/4 دقیقه) بود (05/0p<). فشار داخل چشم نیز به طور متوسط در گروه پروپوفول-کتامین نسبت به گروه پروپوفول کاهش داشت (05/0p<). از طرف دیگر نیاز به داروی آرام بخش اضافی، بیداری و واکنشهای روانی در گروه پروپوفول و کتامین کمتر از گروه پروپوفول بود. از نظر عوارض بالینی 7 نفر در گروه پروپوفول و 6 نفر در گروه پروپوفول-کتامین دچار عوارض شدند که در گروه پروپوفول بیشتر از نوع سرگیجه و در گروه پروپوفول-کتامین از نوع بیقراری بود.
نتیجهگیری: در بیماران تحت بلوک رتروبولبار جهت اعمال کاتاراکت با اضافه کردن کتامین به پروپوفول در مقایسه با پروپوفول، باعث طولانی شدن زمان آرام بخشی و نیز کاهش فشار چشم، بدون افزایش عوارض ناخواسته و مضر در آنها میگردند.

Background and Aim: Retrobulbar block is a relatively unpleasant and painful procedure. For well being and safety of the patients during retrobulbar block, sedation is required. This study was designed to compare the sum of sedative effects of ketamine and propofol with that of propofol alone in patients receiving retrobulbar block for cataract surgery.
Material and Methods: Sixty patients were assigned into two groups randomly and received retrobulbar nerve block for cataract surgery. Under identical conditions, in the propofol group sedation was induced with 0.3mg/kg propofol while sedation in ketamine plus propofol group was induced with 0.3mg/kg propofol plus 0.15mg/kg ketamine. In all patients, vital signs, the status of sedation and hypnoses, intraocular pressure, amnesia, onset and duration of sedation were measured and recorded. Side effects of the drugs were evaluated in the first 24 h after operation. Data were analyzed by means of T-test and x2.
Results: Duration of sedation was longer in ketamine plus propofol group (5.8±2.8 min) than that of propofol group (4.1±2.4 min) (p<0/05(. Intraocular pressure in ketamine plus propofol group was less than that of propofol group (p>0/05). On the other hand ketamine plus propofol group patients showed a decline in the need for excess sedation, awareness, and psychological reactions. Seven patients in propofol group developed vertigo and six patients in ketamine plus propofol group developed agitation as the side effects of these drugs.
Conclusion: Addition of low dose ketamine to propofol will increase duration of sedation and decrease IOP, without increasing unwanted side effects, in comparison with propofol alone for performing retrobulbarblock,.
