fa بررسی وضعیت اپیدمیولوژیک و میزان بروز سرطانها در افراد بالای 15 سال استان کردستان عمومى General پژوهشي اصیل Original Research زمینه و هدف: در حال حاضر بیماری سرطان جزو سه علت اول مرگ و میر در کشورهای توسعه یافته و همچنین ایران است. با توجه به توزیع متفاوت سرطان در دنیا و اهمیت سرطان از نظر ابتلا و مرگ و میر، تعیین الگوی انواع سرطان به عنوان اولین قدم در راه کنترل این بیماری است. این مطالعه به بررسی اپیدمیولوژیک سرطانها در استان کردستان می‌پردازد. روش بررسی: مطالعه به صورت توصیفی و در نمونه‌های در دسترس انجام شد. برای انجام این مطالعه از سیستم ثبت سرطانها که یک برنامه کشوری است استفاده گردید. در سیستم ثبت سرطانها در کشور اطلاعات انواع سرطانهای تشخیص داده شده در مراکز تشـخیص پاتولوژی جمع‌آوری شده و وارد محیط نرم افزاری کشوری ثبت سرطانها شده و کدگذاری نوع سـرطان بر اساسICD-O2 انجام شد. در این مطالعه 1294 مورد سرطان در استان کردستان در سال‌های 1382 و 1383 از کلیه مراکز تشخیص پاتولوژی استان جمع‌آوری و بررسی شد. یافته‌ها: در این مطالعه میزان بروز کل سرطانها در سال‌های 1382 و بترتیب 1383 برابر 60 و 9/66 در یکصد هزار نفر در سال بود. 62 درصد موارد سرطان در مردان و 38 درصد در زنان گزارش شد. در مجموع در هر دو جنس سرطان پوست با 8/22%، سرطان معده با 5/20%، سرطان مری با 8/12%، سرطان مثانه با 5/7% و ریه و برونش با 9/4% سرطانهای شایع بودند. در مردان شایعترین سرطانها به ترتیب شامل معده، پوست، مری، مثانه، ریه، پروستات، کولورکتال و در زنان پوست، مری، معده، پستان و کولورکتال بود. بیشترین میزان بروز سرطان در گروه سنی بالای 75 سال گزارش گردید. بیشترین سرطان پوست از نظر هیستولوژی بازال سل کارسینوما، معده آدنوکارسینوما و مری اسکواموس سل کارسینوما بود. نتیجه‌گیری: از نظر هیستولوژی تفاوتی در انواع سرطان در این مطالعه با دیگر مناطق مشاهده نشد. سیمای اپیدمیولوژیک سرطان در استان کردستان تفاوتهایی با وضعیت آن در کشور دارد که لازم است مورد بررسی بیشتری قرار گیرد. وصول مقاله: 7/9/84 اصلاح نهایی: 24/8/85 پذیرش مقاله: 5/9/85 Background and Aim: At the present time, cancer is among the first three causes of death in developed countries and also in Iran. Considering different distribution of different types of cancers in the world and their significance in regard to morbidity and mortality, diagnosis of different kinds of cancers will be the first step in controlling these disorders. By proper planning, we can reduce the incidence rate of all types of cancers. This survey deals with epidemiological study of cancers in Kurdistan province. Materials and Methods: This was a descriptive study. Data were obtained from cancer registration system, which is a national program. In this system the data of all types of cancers, diagnosed in pathological centers, were collected and entered into the national soft ware program. Different types of cancers were encoded on the basis of ICD_O2. In this study 1294 cases of different types of cancers were collected from pathologic diagnostic centers in Kurdistan province in a period of 2 years (2003-2004). Results: In this survey the overall incidence rate of all types of cancer in 2003 and 2004, was 60 and 66.9 in 100000 people. 62% of cases were male and 38% female. Generally, the most common cancers in both sexes, were those of skin (22.8%), stomach (20.5%), esophagus (12.8%), urinary bladder (7.5%) and lung and bronchus (4.9%). The most common cancers in male patients, belonged to stomach, skin, esophagus, bladder, lung, prostate, colorectal and in female patients, cancers of skin, esophagus, stomach, breast and colorectal carcinoma were the most frequent types, respectively. The highest incidence rate of cancers had occurred in the patients who were older than 75 years. Histopathologically the most common cancers of skin, stomach and esophagus were basal cell carcinoma, adenocarcinoma and squamous cell carcinoma respectively. Conclusion: skin cancers were the most common cancers in Kurdistan province. But the total number of cases of cancers of all parts of Gastro-intestinal tract, exceeded those of all other systems. In Females, opposed to males, esophageal cancer was more common than stomach cancer. Histopathologically, there was no significant difference in the prevalence of different cancers in Kurdistan province, in comparison to other parts of the country.
