fa بررسی میزان تطابق نقش و هویت جنسی 12 بیمار ترنس سکسوال با جنسیت جدیدشان پس ازعمل جراحی تغییر جنسیت عمومى General پژوهشي اصیل Original Research زمینه و هدف: اختلال هویت جنسی تمایل قوی و مستمر بیمار برای قرار گرفتن در نقش و وضعیت جنس مقابل است که اصرار بر تعلق به جنس مقابل در این اختلال همراه با نارضایتی شدید از جنسیت بیولوژیکی می‌باشد. در موارد مقاوم به درمان (روان درمانی فردی، سایکوآنالیز ...) عمل جراحی تغییر جنسیت از درمانهای شناخته شده این اختلال است که در ایران با فتوای امام خمینی در سال 1343 شمسی جایز شمرده شده. با توجه به تحقیقات انجام شده در زمینه بهبود اختلال بعد از عمل تغییر جنسیت غالباً بر نتایج مثبت جراحی تأکید شده است. روش بررسی: روش تحقیق در این پژوهش پیمایشی مقطعی در مقیاس کوچک بوده است. نمونه مورد بررسی شامل 12 نفر (9 نفر ترنس مرد به زن و 3 نفر دیگر ترنس زن به مرد) بوده که به طور متوسط 6 سال از عمل جراحی شان سپری شده بود. پرسشنامه‌های M‏MPI-1 (فرم کوتاه)، مقیاس (MF)MMPI-2 ، نقش جنسی بم و صفات جنسی بم با نظارت یک روانشناس بالینی و توسط بیماران تکمیل شد. یافته‌ها با استفاده از روشهای آمار توصیفی مورد تجزیه و تحلیل آماری قرار گرفت. یافته‌ها: نتایج نشان می‌دهد که این بیماران بعد از عمل جراحی از نظر روانی- اجتماعی در وضعیت مناسبی قرار داشته و نمره کسب شده آنان درMMPI- 1 در دامنه نرمال بود. میانگین سنی آزمودنی‌ها 25 سال و میانگین وضعیت مالی خانوادگی آنان در حد متوسط بود. همچنین افراد مذکور در تستهای بم و مقیاس MF تطابق خوبی را با نقش و هویت جدید خود نشان دادند. مصاحبه بالینی نیمه سازمان یافته نشان داد که تمامی افراد مورد مطالعه از تصمیم خود رضایت داشته موردی از پشیمانی نیز مشاهده نشد. نتیجه‌گیری: به نظر می‌رسد عوامل متعدد از جمله وضعیت اقتصادی، رضایت از روابط بین فردی، وجود سیستم حمایتی فعلی، شریک جنسی و رضایتمندی از زندگی جنسی، عکس‌العمل مناسب خانواده و امکان انجام عمل‌های جراحی اضافه، نقش عمده‌ای در تطابق بیماران با وضعیت جدید داشته است. کلید واژه‌ها: اختلال هویت جنسی، ترنس سکسوالیسم، نقش و هویت جنسی، عمل جراحی تغییر جنسیت، رضایت بعد از عمل وصول مقاله: 8/3/87 اصلاح نهایی: 20/6/87 پذیرش مقاله: 24/6/87 Background and Aim: Gender identity disorder consists of the patient’s consistent desire to identify with the opposite sex and feels significant discomfort or the inability to deal with this condition. In patients resistant to treatment (Individual Psychotherapy, Psychoanalysis …) operational procedure has been utilized and has been authorized in Iran through a religious decree issued by the country’s late leader, Ayatollah Khomeini since 1964. A study into the instances of surgery procedures in the country has proven positive results. Material and Methods: The procedure consists of cross-sectional study in small scale on 12 transsexual individuals consisting of 9 men and 3 women. All of the participants had sexual reassignment surgery at an average of six years prior to the study. They all were from middle class families and held an average age of 25. The participants were subjected to MMPI-1 questionnaire, (MF) MMPI-2, The Bem Sex-Role inventory and the Bem Gender Traits Test. Results: The study showed that the patients have responded positively to the treatment in psycho –social dimension and had obtained a normal grade at the MMPI- 1 test. According to the results of the Bem and MF tests they have also accepted their new identity and role. Results of Semi structural interview included all of sample agreed with their decision. Neither of the patients has expressed regret of undertaking the surgery Conclusion: Multiple factors include economic status of the patients, satisfying interpersonal relationships, efficient support system, sexual partners and satisfactory sexual life, suitable familial reactions and possibility of extra surgery have prominent role to adjust them with their new identity and role in the society.
