fa مقایسه اثر قبل با بعد از تزریق داخل کورپوس کاورنوزوم Platelet-rich plasma در بهبود عملکرد جنسی مردان با اختلال نعوظ Comparison of the effect before and after intracorpus cavernosum Platelet-rich plasma injection in imporoving sexual function in men with erectile dysfunction پزشکی- اورولوژی Medicine - Urology پژوهشي اصیل Original Research زمینه و هدف: اختلال نعوظ (ED) یک مشکل رایج در بین مردان است و استفاده از درمان پلاسمای غنی از پلاکت  (PRP) برای درمان ED توجه فزاینده‌ای را به خود جلب کرده است، اما هنوز هیچ مدرک قطعی در مورد اثربخشی آن وجود ندارد. هدف از انجام این مطالعه مقایسه اثر  قبل با بعد از تزریق داخل کورپوس کاورنوزوم PRP در بهبود عملکرد جنسی مردان با اختلال نعوظ بوده است. مواد و روش ها: این مطالعه مداخله‌ای قبل و بعد تک‌بازویی بدون هیچ تصادف سازی است که بر روی 32 نفر از مردان دارای  اختلال نعوظ مراجعه کننده به بیمارستان کوثر سنندج انجام شد.  در این مطالعه در ابتدا پس از تکمیل مشخصات دموگرافیک نمره اختلال عملکرد جنسی آنها با استفاده از  پرسشنامه IIEF  اندازه گیری شد. سپس افراد PRP  در روزهای صفر و 20 و40و 60 دریافت هربار پرسشنامه را پر کردند.  ارزیابی شاخص های پیامد20روز بعد و 40روز بعد و 60 روز بعد  از دریافت PRP انجام AN. مهمترین شاخص پیامد عملکرد جنسی بیماران است که با استفاده از پرسشنامه زیر (IIEF) اندازه گیری شد. برای تجزیه و تحلیل داده ها، با توجه به کیفی بودن متغیر وابسته از آزمون های تی،کای اسکوئر و رگرسیون لجستیک استفاده شد. برای تمام آزمون ها سطح معناداری برابر 0.05 در نظر گرفته شد. نتایج : نتایج مطالعه نشان داد که میانگین سن بیماران  57.31 سال بوده است. نتایج نشان داد که میانگین امتیاز کلی قبل از تزریق 2.63±7.43، بیست روز بعد تزریق  3.53±9.87، چهل روز بعد تزریق 3.52±10.81و میانگین امتیاز کلی شصت روز بعد تزریق3.81 ± 11.46بوده است. نتایج تحلیلی نشان داد بین امتیاز نعوظ کافی قبل و بعد از تزریق (20،40،60 روز) ، نعوظ کافی برای دخول، نعوظ کافی بعد از دخول قبل از تزریق و بیست روز بعد از تزریق تفاوت معنی دار وجود داشته است. همچنین  بین میانگین امتیاز کلی قبل از تزریق و  بعد از تزریق (روز20 ،40 و 60) تفاوت معنی دار وجود داشته است و بنظر می رسد تزریق داخل کورپوس کاورنوزوم PRP   در بازیابی عملکرد نعوظ طبیعی و میزان رضایت عملکرد جنسی افراد مورد مطالعه تاثیر بسزایی داشته است. (0.0001 P<). اما بین امتیاز  حفظ نعوظ تا کامل شدن رابطه قبل از تزریق و بعد از تزریق تفاوت معنی دار آماری مشاهده نشده است(0.05 P>).  بحث : درمان PRP برای اختلال نعوظ، اثربخشی امیدوارکننده‌ای را نشان داده است و پتانسیل تبدیل شدن به یک گزینه درمانی جایگزین جدید برای بیماران مبتلا به اختلال نعوظ را دارد Background and Aim: Erectile dysfunction (ED) is a common problem among men, and the use of platelet-rich plasma (PRP) therapy for the treatment of ED has attracted increasing attention, but there is still no definitive evidence of its effectiveness. The aim of this study was to compare the effect before and after intracorpus cavernosum PRP injection in improving sexual function in men with erectile dysfunction. Materials and methods: This is a single-arm, before-and-after interventional study without any randomization was conducted on 32 men with erectile dysfunction who referred to Kowsar Hospital in Sanandaj. In this study, after completing demographic characteristics, their sexual dysfunction score was measured using the IIEF questionnaire. Then, the PRP subjects filled out the questionnaire on days 0, 20, 40, and 60 after receiving each. Evaluation of outcome indicators 20 days later, 40 days later, and 60 days after receiving PRP was performed. The most important outcome indicator is the sexual function of the patients, which was measured using the following questionnaire (IIEF). For data analysis, considering the qualitative nature of the dependent variable, t-test, chi-square, and logistic regression tests were used. For all tests, a significance level of 0.05 was considered. Results: The results of the study showed that the average age of the patients was 57.31 years. The results showed that the average total score before injection was 7.43± 2.63, twenty days after injection was 9.87±3.53, forty days after injection was 10.81±3.52, and the average total score sixty days after injection was 11.46±3.81. The analytical results showed that there was a significant difference between the score of sufficient erection before and after injection (20, 40, 60 days), sufficient erection for penetration, sufficient erection after penetration before injection and twenty days after injection. There was also a significant difference between the average total score before injection and after injection (days 20, 40, and 60), and it seems that intracorpus cavernosum PRP injection had a significant effect on restoring normal erectile function and satisfaction with sexual function of the subjects studied. (P<0.0001). However, no statistically significant difference was observed between the score of maintaining erection until completion of intercourse before and after injection (P>0.05).
