fa ارزیابی چالش های برنامه شناسایی اختلالات روانپزشکی در مراکز سلامت شهری: یک مطالعه بر اساس مدل CIPP Evaluation of challenges of the psychiatric disorder identification program in urban health centers: A study based on the CIPP model پزشکی خانواده و اجتماعی Family and Social Medicine پژوهشي اصیل Original Research ززمینه و هدف: این مطالعه با هدف ارزیابی برنامه غربالگری و شناسایی اختلالات روانپزشکی در جمعیت تحت پوشش مراکز خدمات جامع سلامت شهری انجام شد.  مواد و روش ها: در این مطالعه ترکیبی کمی-کیفی، شاخص‌های موجود برنامه غربالگری و شناسایی اختلالات روانپزشکی با شاخص‌های ملی در جمعیت تحت پوشش مراکز خدمات جامع سلامت دانشگاه شهید بهشتی (<a name="_Hlk229817647"></a><a name="_Hlk229817074">شامل تهران</a>T شمیرانات، قرچک، پاکدشت، ورامین، پیشوا، پردیس، دماوند و فیروزکوه) مقایسه شد. داده‌ها از طریق مصاحبه گروهی نیمه‌ساختاریافته با 25 شرکت‌کننده انتخاب‌شده به صورت هدفمند جمع‌آوری گردید. تجزیه و تحلیل داده‌ها بر اساس مدل ارزیابی CIPP (زمینه، درونداد، فرآیند، برونداد) در سه حیطه (درونداد، فرآیند، زمینه) انجام شد و اعتبار داده‌ها با استفاده از معیارهای لینکلن و گوبا تأیید گردید. یافته ها: در بخش کمی، تنها شاخص‌های حیطه فرآیند با شاخص‌های ملی تفاوت واضحی داشتند. شاخص های حیطه درونداد کاملا مطلوب و شاخص های حیطه برونداد تا حدود زیادی مطلوب بود. در بخش کیفی، عمده ترین چالش های موجود در حیطه ها به ترتیب  عوامل مربوط به خانوار و تعیین‌کننده‌های دریافت خدمات در حیطه زمینه،  منابع انسانی در حیطه درونداد و آموزش پرسنل بودند. یافته‌های کیفی به خوبی چالش‌های مشاهده شده در حیطه فرایند و برونداد در نتایج کمی را تبیین نمودند. نتیجه گیری: نتایج نشان می‌دهد که بهتر است برنامه غربالگری و شناسایی اختلالات روانپزشکی در کنار گسترش پوشش خدمات به سمت ارتقای کیفیت خدمات و رضایت جمعیت نیز سوق داده شود. همچنین به منظور دستیابی به نتایج مطلوب تر در برنامه غربالگری روانپزشکی رفع مشکلات اجرایی اشاره شده به آنها اهمیت به سزایی دارد.   Background and Aim: This study aimed to evaluate the screening and identification program for psychiatric disorders among the population covered by urban comprehensive health centers. Materials and Methods: In this mixed-methods study, existing indicators of the psychiatric disorder screening and identification program were compared with national benchmarks in the population covered by comprehensive health centers under Shahid Beheshti University (covering Tehran, Shemiranat, Qarchak, Pakdasht, Varamin,Pishva, Pardis, Damavand and Firoozkouh). Data were collected through semi-structured group interviews with 25 purposively selected participants. Analysis followed the CIPP (Context, Input, Process, Product) evaluation model across three domains (Input, Process, Context), with data validity verified using Lincoln and Guba’s criteria. Results: In the quantitative section, only the process domain indicators had a clear difference with the national indicators. The indicators of the input domain were completely desirable and the indicators of the output domain were largely desirable. In the qualitative section, the main challenges in the domains were household factors and determinants of service receipt in the context domain, human resources in the input domain, and personnel training, respectively. The qualitative findings well explained the challenges observed in the process and output domains in the quantitative results. Conclusion: Results suggest that it is better to direct the screening and identification program for psychiatric disorders, in addition to expanding service coverage, towards improving the quality of services and population satisfaction. Also, in order to achieve more desirable results in the psychiatric screening program, it is of great importance to resolve the implementation problems mentioned. اختلالات روانپزشکی، سلامت روان، برنامه های سلامت، الگوی ارزشیابی CIPP.
