fa تحلیل سیاست گذاری برنامه مراقبت در منزل در ایران پرستاری Nursing پژوهشي اصیل Original Research <span style="font-size:12pt"><span style="text-justify:kashida"><span style="text-kashida:0%"><span style="line-height:normal"><span style="direction:rtl"><span style="unicode-bidi:embed"><span new="" roman="" style="font-family:" times=""><b><span b="" lang="FA" style="font-family:" zar="">زمینه و هدف:</span></b><span b="" lang="FA" style="font-family:" zar=""> مراقبت در منزل به‌عنوان یکی از روش‌های مراقبت مبتنی بر جامعه و از اشکال کلیدی مراقبت سلامت مطرح است که در مدیریت منابع نقش مهمی ایفا می‌کند؛ لذا، </span><span b="" lang="AR-SA" style="font-family:" zar="">مطالعه </span>&nbsp;<span b="" lang="AR-SA" style="font-family:" zar="">حاضر</span> <span b="" lang="AR-SA" style="font-family:" zar="">باهدف نحوه</span> <span b="" lang="AR-SA" style="font-family:" zar="">اجرای سیاست مراقبت در منزل در ایران انجام</span> <span b="" lang="AR-SA" style="font-family:" zar="">گردید</span>.<span style="font-family:"B Zar"" lang="FA"></span></span></span></span></span></span></span></span><br> <span style="font-size:12pt"><span style="text-justify:kashida"><span style="text-kashida:0%"><span style="line-height:normal"><span style="direction:rtl"><span style="unicode-bidi:embed"><span new="" roman="" style="font-family:" times=""><b><span b="" lang="FA" style="font-family:" zar="">مواد و روش&shy;ها: </span></b><span b="" lang="FA" style="font-family:" zar="">پژوهش حاضر از دسته مطالعات تحلیل سیاست است&nbsp; که بر اساس چارچوب مثلث تحلیل سیاست والت و گیلسون انجام گرفت. جهت تحلیل برنامه مراقبت در منزل در ایران از روش‌های مختلفی از جمله مرور متون، بررسی اسناد و مدارک استفاده گردید.&nbsp; در مرحله بعدی مطالعه و جهت معتبرسازی یافته‌های قبلی مطالعه کیفی به شیوه تحلیل محتوای هدایت شده انجام شد.<b> </b></span></span></span></span></span></span></span></span><br> <span style="font-size:12pt"><span style="text-justify:kashida"><span style="text-kashida:0%"><span style="line-height:normal"><span style="direction:rtl"><span style="unicode-bidi:embed"><span new="" roman="" style="font-family:" times=""><b><span b="" lang="FA" style="font-family:" zar="">&nbsp;یافته‌ها:</span></b><span b="" lang="FA" style="font-family:" zar=""> بررسی نحوه اجرای سیاست مراقبت در منزل در ایران نشان داد که اغلب نقش‌آفرینانی که در استقرار و اجرای برنامه نقش داشته‌اند عملکردشان در جهت ارتقای مراقبت در منزل در ایران &nbsp;مناسب نبوده است. مواردی از قبیل </span><span b="" lang="AR-SA" style="font-family:" zar="">وجود سیاست‌گذاری و برنامه‌ریزی‌های &nbsp;نامناسب، مسائل قانونی و اخلاقی، </span><span b="" lang="FA" style="font-family:" zar="">هزینه‌های بالای خدمات مراقبت در منزل برای بیماران</span><span b="" lang="AR-SA" style="font-family:" zar="">، &nbsp;</span><span b="" lang="FA" style="font-family:" zar="">عدم پوشش هزینه‌های خدمات مراقبتی توسط سازمان‌های بیمه‌گر، سیاست‌های مالی ناکارآمد برای حمایت از بیماران </span><span b="" lang="AR-SA" style="font-family:" zar="">و غیره نیازمند مداخلات سریع هستند</span><span b="" lang="FA" style="font-family:" zar="">.</span></span></span></span></span></span></span></span><br> <span style="font-size:12pt"><span style="line-height:normal"><span style="direction:rtl"><span style="unicode-bidi:embed"><span new="" roman="" style="font-family:" times=""><b><span b="" lang="FA" style="font-family:" zar="">نتیجه‌گیری:</span></b> <span b="" lang="AR-SA" style="font-family:" zar="">اجرایی‌کردن مناسب &nbsp;طـرح مراقبت در منزل &nbsp;نیازمنـد تعریف آن در نظـام سـلامت ایـران، شـفاف و عملیاتـی نمـودن پوشـش بیمـه همگانـی، برنامه‌ریزی جهت ارزیابی و رفع چالش‌ها و موانع پیشرو، اصلاحـات برنامـه درسـی پرسـتاری و همچنیـن </span>&nbsp;<span b="" lang="AR-SA" style="font-family:" zar="">اطمینان‌بخشی بــه آحــاد جامعــه در زمینــه مزایــای اجــرای این طرح است؛ لذا</span> <span b="" lang="FA" style="font-family:" zar="">مناسب‌سازی آموزش‌های موردنیاز در دوره کارشناسی پرستاری و توجه به منافع همه ذی‌نفعان می‌تواند در استقرار کامل این برنامه در سطح بیمارستان‌ها مؤثر باشد. </span></span></span></span></span></span><br> &nbsp; <span style="font-size:12pt"><span style="line-height:normal"><span style="unicode-bidi:embed"><span new="" roman="" style="font-family:" times=""><b>Background and Aim:</b> Home care is considered as one of the community-based care methods and a key form of health care that plays an important role in resource management. Therefore, the present study aimed to <span style="color:red">determine</span> the implementation of home care policy in Iran.</span></span></span></span><br> <span style="font-size:12pt"><span style="line-height:normal"><span style="text-autospace:none"><span style="unicode-bidi:embed"><span new="" roman="" style="font-family:" times=""><b>Materials and Methods:<i> </i></b>The present study is a policy analysis study conducted retrospectively and based on the Walt and Gilson policy analysis triangle framework. Various methods were used to analyze the home care program in Iran, including literature review and document review. In the next stage of the study, and to validate the previous findings, a qualitative study was conducted using a guided content analysis method.</span></span></span></span></span><br> <span style="font-size:12pt"><span style="line-height:normal"><span style="text-autospace:none"><span style="unicode-bidi:embed"><span new="" roman="" style="font-family:" times="">.</span></span></span></span></span><br> <span style="font-size:12pt"><span style="line-height:normal"><span style="text-autospace:none"><span style="unicode-bidi:embed"><span new="" roman="" style="font-family:" times=""><b>Results:<i> </i></b>A study of the implementation of home care policy in Iran showed that most of the actors who played a role in establishing and implementing the program did not perform well in promoting home care in Iran. Cases such as inappropriate policy-making and planning, legal and ethical issues, high costs of home care services for patients, non-coverage of care services by insurance organizations, inefficient financial policies to support patients, etc. require rapid interventions.</span></span></span></span></span><br> <span style="font-size:12pt"><span style="line-height:normal"><span style="unicode-bidi:embed"><span new="" roman="" style="font-family:" times=""><b>Conclusion: </b>Proper implementation of the home care plan requires its definition in the Iranian health system, making public insurance coverage transparen and operational, planning to assess and address challenges and obstacles, nursing curriculum reforms, and also assuring the community of the benefits of implementing this plan.
