fa مقایسه رادیوبیولوژیک و دوزیمتریک دو رژیم استاندارد وهایپوفراکشنه در پرتودرمانی سرطان پستان چپ با تکنیک شدت مدوله شده مستقیم: تحلیل منحنی‌های دوز-حجم و احتمال عوارض بافت‌های سالم Radiobiological and Dosimetric Comparison of Standard and Hypofractionated Regimens in Left-Sided Breast Cancer Radiotherapy Using Forward Intensity-Modulated Technique: Analysis of Dose-Volume Curves and Normal Tissue Complication Probability فیزیک پزشکی Medical Physics پژوهشي اصیل Original Research  زمینه و هدف: این مطالعه گذشته نگر به منظور مقایسه دو رژیم درمانی استاندارد (50 گری در 25 جلسه) و درمان هیپوفرکشنه (42/56 گری در 16 جلسه)، در پرتودرمانی با شدت مدوله شده مستقیم سرطان پستان چپ با تمرکز بر پارامترهای دوزیمتریک و رادیوبیولوژیک را با استفاده از یک مدل رادیوبیولوژیک طراحی شده است. مواد و روشها: تعداد 103 بیمار مبتلا به سرطان پستان چپ که غدد لنفاوی فوق‌ترقوه‌ای و زیر بغل آن‌ها درگیر نبودند انتخاب شدند. طراحی درمان با یک گروه شامل 53 بیمار که رژیم فرنکشنال استاندارد (SF) را دریافت می‌کردند و یک گروه دیگر از 50 بیمار که تحت رژیم هیپوفرکشنال (HF) قرار داشتند، انجام شد. پارامترهای رادیوبیولوژیک مورد تجزیه و تحلیل بر اساس مدل Niemierko شامل: دوز یکنواخت معادل(EUD)، احتمال کنترل تومور (TCP)، احتمال عوارض بافت نرمال (NTCP) و پارامترهای دوزیمتریک مانند شاخص همگنی (DHI) هدف، دوز میانگین (Dmean)، حجم قلب دریافت‌کننده 30 گری (V30Gy) و حجم ریه دریافت‌کننده 20 گری (V20Gy) بود. یافته‌ها: تحلیل آماری با استفاده از آزمون تی جفت‌شده نشان داد که تفاوت معناداری در پارامترهای DHI و TCP بین دو رژیم درمانی وجود ندارد. دوز میانگین (Dmean) برای ریه و قلب تفاوت معناداری بین دو رژیم نشان نداد .(p > 0/05) همچنین، تفاوت معناداری در V20Gy برای ریه و V30Gy برای قلب مشاهده نشد .(p > 0/05) مدل NTCP بر اساس مدل نی‌میرکو ریسک عوارض قلبی راپیش بینی نکرد. با این حال، NTCP   ریه در تکنیک HF به‌طور معناداری نسبت به تکنیک SF کاهش یافت  .(p < 0/05) همچنین، EUD ریه در رژیم HF به‌طور معناداری کمتر از رژیم SF بود5 /16 گری در برابر 10/5 گری (p = 0/04)، در حالی‌ که EUD قلب بین دو گروه تفاوت معناداری را نشان نداد (p = 0/26) . نتیجه‌گیری: این مطالعه تفاوت معنیداری را در پارامترهای دوزیمتریک و رادیوبیولوژیک از نظر احتمال کنترل تومور بین درمان‌های هایپوفرکشنال و استاندارد برای سرطان پستان چپ نشان می‌دهد. با این حال، رژیم‌های HF در مقایسه با  SFتفاوت معنی‌داری در کاهش عوارض ریوی ناشی از تابش نشان داد. Background and Aim: This retrospective study investigates and compares two treatment regimens, namely, standard therapy (50 Gy in 25 fractions) and hypofractionated therapy (42.56 Gy in 16 fractions), in forward intensity modulated radiation therapy (FIMRT) for left breast cancer, focusing on dosimetric and radiobiological parameters using a radiobiological model. Materials and Methods: A total of 103 patients with left breast cancer, with no involvement of supraclavicular and axillary lymph nodes (N0 stage), were selected. The treatment design was performed with one group consisting of 53 patients receiving the standard fractionation (SF) regimen and another group of 50 patients undergoing the hypofractionated (HF) regimen. Analyzed radiobiological parameters based on the Niemierko model included: equivalent uniform dose (EUD), tumor control probability (TCP), normal tissue complication probability (NTCP), and dosimetric parameters such as homogeneity index (DHI) of the target, mean dose (Dmean), volume of the heart receiving 30 Gy (V30Gy), and volume of the lung receiving 20 Gy (V20Gy). Results: Statistical analysis using the paired t-test showed no significant differences in DHI and tumor control probability (TCP) between the two treatment regimens. The mean dose (Dmean) to the lung and heart did not differ significantly between the standard fractionation (SF) and hypofractionation (HF) groups (p > 0/05). Similarly, no significant differences were observed in lung V20Gy and heart V30Gy (p > 0/05). According to the Niemierko NTCP model, the risk of cardiac toxicity was negligible. However, lung NTCP was significantly lower in the HF group compared to the SF group (p < 0/05). Furthermore, EUD to the lung was significantly lower in the HF regimen than in the SF regimen (10/5 Gy vs. 16/5 Gy; p = 0/04), whereas the EUD to the heart showed no significant difference between the two groups (p = 0/26). Conclusion: This study demonstrated no significant differences in dosimetric and radiobiological parameters related to tumor control probability between hypofractionated and conventional radiotherapy regimens for left-sided breast cancer.
