fa ارتباط بین پلی‌مورفیسم ژن عامل رشد اندوتلیال عروقی (VEGF-2578 C/A; rs699947) با پروفایل لیپید در بیماران ایرانی مبتلا به آرتریت روماتوئید The association between vascular endothelial growth factor gene polymorphism (VEGF-2578 C/A; rs699947) and lipid profile in Iranian patients with rheumatoid arthritis پزشکی مولکولی و ژنتیک Molecular Medicine and Genetics پژوهشي اصیل Original Research زمینه و هدف:  آرتریت روماتوئید (RA)، یک بیماری مزمن خود ایمنی با علل ژنتیکی و محیطی است. شواهد حاکی از افزایش خطر ایجاد آترواسکلروز و مرگ و میر ناشی از آن در بیماران مبتلا به RA هستند. عامل رشد اندوتلیال عروقی (VEGF)، مهم ترین مولکول پروآنژیوژنیک است که هم در پاتوژنز  RA  و هم در روند آترواسکلروز نقش مهمی را ایفا می کند. ژن VEGF  بسیار پلی مورفیک است. بنابراین هدف مطالعه حاضر، تعیین نقش پلی‌مورفیسم 2578- ژن VEGF (rs699947) در تغییر پروفایل لیپید در بیماران نژاد کرد مبتلا به RA بوده است. روش کار: در این مطالعه مقطعی که در سال 1399 انجام گردید، 130 بیمار مراجعه‌کننده به درمانگاه روماتولوژی بیمارستان توحید سنندج که تشخیص آرتریت روماتویید در آنها مسجل شده بود، وارد مطالعه شدند و جهت تعیین درد در آن ها از شاخص VAS استفاده شد. مقادیر ESR (سرعت رسوب گلبولهای قرمز خون)، CRP (پروتئین واکنشگر C) و RF و پروفایل لیپیدی بیماران شامل سطوح پلاسمایی تری گلیسرید، کلسترول تام و کلسترول HDL-C (لیپوپروتئین با چگالی بالا) ثبت و شدت بیماری بر اساس معیار DAS28 محاسبه شد. همچنین پلی‌مورفیسم موقعیت 2578- ژن VEGF توسط PCR-RFLP سنجش گردید. یافته‌ها: توزیع فراوانی پلی‌مورفیسم‌های CC، AA و AC و فرآوانی آلل‌های ژن VEGF در نقطه rs699947 با مقدار قابل انتظار تعادل هاردی-واینبرگ سازگار بود. فراوانی ژنوتیپ‌های AA، AC و CC به ترتیب 3/42%، 8/43% و 8/13% بود. ژنوتیپ CC موقعیت 2578- ژن VEGF، ارتباط معکوسی را با شاخص آتروژنیک (01/0P = ) و نیز سطوح کلسترول تام (01/0 P =) نشان داد. بین پلی‌مورفیسم موقعیت 2578- ژن VEGF و دیگر پارامترهای لیپید، CRP، RF و شدت بیماری، رابطه معنی‌داری مشاهده نشد (05/0 <P ). نتیجه‌گیری: یافته‌های ما نشان می‌دهد که تغییرات ژنتیکی در ژن VEGF با پروفایل لیپیدهای پلاسما، از جمله سطح کلسترول تام و شاخص آتروژنیک، در بیماران مبتلا به آرتریت روماتوئید مرتبط است و ممکن است از طریق ارتباط با متابولیسم لیپیدها در بروز عوارض مرتبط با RA نقش داشته باشد.   Background: Rheumatoid arthritis (RA) is a chronic autoimmune disease with a genetic and environmental etiology. Evidence suggests an increased risk of atherosclerosis and its associated mortality in patients with RA. Vascular endothelial growth factor (VEGF) is the most important pro-angiogenic factor implicated in the pathogenesis of both RA and atherosclerosis. Therefore, this study aimed to determine the role of the VEGF -2578 polymorphism (rs699947) in changes in lipid profiles in Kurdish patients with RA. Methods: In this cross-sectional study, conducted in 2020, 130 patients with a confirmed diagnosis of rheumatoid arthritis who were referred to the Rheumatology Clinic of Tohid Hospital in Sanandaj were enrolled. Pain severity was assessed using the Visual Analog Scale (VAS). Plasma lipid profiles, including triglycerides (TG), total cholesterol (TC), and HDL-cholesterol, were measured by an auto-analyzer. Erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), and rheumatoid factor (RF) values were recorded, and the severity of disease was calculated according to disease activity score-28 (DAS28) criteria. VEGF-2578 polymorphism was also genotyped by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP).  Results: The genotype distributions of AA, AC, and CC and allele frequencies for the VEGF-2578 at rs699947 did not show any significant difference from the expected value of the Hardy-Weinberg equilibrium. The frequencies of AA, AC, and CC genotypes were 42.3%, 43.8%, and 13.8%, respectively. The VEGF-2578 CC genotype was shown to be inversely associated with atherogenic index (P = 0.01) and total cholesterol levels (P = 0.01). No significant relationship was found between the VEGF-2578 polymorphism and other lipid parameters, CRP, RF, and disease severity (P > 0.05). Conclusions: Our findings indicate that genetic variation in the VEGF gene is associated with plasma lipid profiles, including total cholesterol levels and the atherogenic index, in patients with RA, and may have implications for RA-related complications via its relationship with lipid metabolism.   آرتریت روماتوئید؛ پلی موفیسم VEGF؛ آترواسکلروز؛ پلی مورفیسم ژنتیکی.
